摘 要:目的比较纳洛酮或肾上腺素对窒息性心跳骤停大鼠心肺复苏的效果。方法健康SD大鼠24只,体重180~220g,雌雄不拘,制备窒息性心跳骤停模型,随机分成3组(n=8):对照组在常规心肺复苏(CPR)前静脉注射生理盐水1ml,纳洛酮组和肾上腺素组在常规CPR前分别静脉注射纳洛酮1mg/kg和肾上腺素0.04mg/kg。记录在CPR 10min内自主循环恢复(ROSC)情况。结果对照组、纳洛酮组和肾上腺素组ROSC率分别为12.5%、87.5%和87.5%,纳洛酮组和肾上腺素组ROSC率均高于对照组(P〈0.05),纳洛酮组和肾上腺素组间ROSC率差异无统计学意义(P〉0.05)。结论静脉注射纳洛酮1mg/kg或肾上腺素0.04mg/kg对窒息性心跳骤停大鼠均可有效地进行心肺复苏,且其心肺复苏的早期效果一致。Objective To investigate the effects of naloxone and epinephrine on the outcome of cardiopulmonary resuscitation (CPR) in a rat model of asphyxial cardiac arrest.methods Twenty-four SD rats of both sexes weighing 180-220 g were randomized into 3 groups ( n = 8 each) : Ⅰ control group (C) received normal saline 1 ml i.v. ; Ⅱ naloxone group (N) received naloxone 1 mg/kg intravenously and Ⅲ epinephrine group (E) received epinephrine 0.04 mg/kg intravenously at the end 10 rain asphyxia when CPR was started. The animals were anesthetized with intraperitoneal urethane 1 g/kg, tracheostomized and mechanically ventilated (VT = 6 ml/kg, RR = 70 bpm). Left carotid artery and right external jugular vein were cannulated for monitoring of BP and right atrial pressure. ECG was continuously monitored using needle electrodes. CPR consisted of closed cardiac massage and mechanical ventilation. Resuscitation efforts were discontinued if it failed to restore spontaneous circulation within 10 rain. The success rate of restoration of spontaneous circulation (ROSC) was compared among the 3 groups.Results The rate of ROSC was 12.5% (in control group), 87.5% (in naloxone group) and 87.5% (in epinephrine group) respectively, and was significantly higher in group N and group E than in control group. Conclusion Naloxone 1 mg/kg i .v. is as effective as epinephrine (0.04 mg/kg i.v. ) in CPR.
