两种超声心动图诊断老年慢性心功能衰竭标准的评价与比较

摘　　要：目的评价不同超声心动图标准诊断老年慢性心力衰竭(CHF)的价值。方法按照NYHA临床心功能分级,心功能Ⅰ级的31例患者作为心功能正常组,心功能Ⅱ到Ⅳ级的288例患者作为CHF组。以左室射血分数(LVEF)和左室舒张早期与晚期血流速度比值(E/A)作为指标,对欧洲心脏病学会(ESC)及我国以往超声诊断心衰标准对老年临床CHF的诊断价值进行评价。结果欧洲心脏病学会及我国以往超声标准诊断老年临床CHF的价值分别为:灵敏度44%vs83%(P<0.01),特异度100%vs81%(P<0.05),准确度49%vs83%(P<0.01)。结论国内既往超声心动图的诊断标准较ESC标准可提高对中国老年CHF的检出率,与NYHA分级符合率高。有利于老年CHF患者的早发现、早诊断和早干预。AIMTo evaluate the diagnostic value of European Society of Cardiology （ ESC ） criteria and Chinese criteria for echocardiogram in the elderly CHF. METHODS Thirty-one patients with NYHA class I served as normal group and 288 patients with NYHA class Ⅱ-Ⅳ as heart failure group. The ages of the subjects were 〉t60 years old. The left ventricular ejection fraction（ LVEF）, the transmitral peak rapid filling velocity （E）, peak atrial filling velocity （A） and the E/A ratio were taken as echocardiogram parameters to evaluate and compare the diagnostic value of ESC criteria and Chinese criteria in elderly CHF patients. RESULTS The sensitivity, specificity and accuracy of ESC criteria and Chinese criteria were respectively44% vs83% （P〈0.01）, 100% vs81% （P〈0.05） and49% vs83% （P〈0. 01 ） in diagnosing elderly CHF. CONCLUSIONS Compared with ESC criteria, Chinese echocardiogram criteria of elderly CHF has a higher diagnostic rate, which helps early diagnosis and treatment of CHF in elderly patients.
