磁共振不同序列显示内听道段面神经及血管关系的对比研究

摘　　要：目的探讨不同序列在显示内听道段面神经及血管关系的优劣势,找到最佳的显示序列。方法筛选10例正常志愿者(无临床症状的面神经周围有血管袢者)和20例面肌痉挛患者,分别使用T2WI/3D/TSE、B-TFE和T1W I/3D/FFE序列进行同一部位的内耳轴位扫描。3个序列均使用相同的层数、层厚和层间距。将每一个层面得出的3种图像进行比较评级。结果在显示面神经方面T2WI/3D/TSE和B-TFE都有优势,两者没有显著差异(P>0.05);而T1WI/3D/FFE对面神经的显示不如前两者(P<0.05),有显著差异。在显示血管袢方面T2WI/3D/TSE明显优于后两者(P<0.05),有显著差异。结论T2WI/3D/TSE序列是目前显示内听道段面神经和血管袢关系的最佳序列,单纯观察神经可选B-TFE序列。Objective To evaluate the advantages and disadvantages of different MR sequences in showing the relationship between facial nerve and blood vessels in internal acoustic meatus, and to look for the best sequence of showing internal acoustic meatus. Methods 10 volunteers and 20 cases with facial spasm were examined on the same axial position of the internal acoustic meatus with T2WI/3D/TSE, B -TFE and T1 WI/3D/FFE. The slices, slice thickness and slice interval were the same in different sequences. Then the same slices from the three different sequences were compared with rank . Results Facial nerve was well displayed with T2WI/3D/TSE and B - TFE . There was no significant difference between them （P 〉 0.05 ） . However , T1 WL/3D/FFE was inferior to T2WI/3D/TSE and B -TFE , with significant difference between them IP 〈 0.05 ）. In showing blood vessels , T2WI/3D/TSE was superior to B- TFE and T1 WI/3D/FFEIP 〈 0.05 ）. Conclusion T2WI/3D/TSE sequence is the best sequence in showing facial nerve and blood vessels in internal acoustic meatus. B - TFE sequence can be used when only to show facial nerve.
