摘 要:结扎兔冠脉左室枝45min,再灌注180min,复制急性心肌缺血/再灌注模型,观察等容血液稀释和超氧化物歧化酶(SOD)对兔心肌缺血/再灌注损伤保护的协同作用并探讨其可能的机制。40只动物随机分为4组:Ⅰ组(对照组)、Ⅱ组(稀释组)、Ⅲ组(SOD组)和Ⅳ组(稀释+SOD组)。结果:与Ⅰ组比,Ⅱ,Ⅲ组左室收缩功能(最大收缩压、+dp/dtmax)显著改善,Ⅳ组较Ⅱ,Ⅲ组又有显著改善。与Ⅰ组梗塞面积(左室梗塞面积占左室总面积的百分比为18.1±2.3%)比,Ⅱ,Ⅲ,Ⅳ组梗塞面积显著缩小(分别为11.4±2.1%,13.8±2.2%,8.3±2.9%),Ⅳ组梗塞面积较Ⅲ组又有进一步缩小(P<0.05)。提示:等容血液稀释和SOD对兔缺血/再灌注心肌均有保护作用;两者联用可进一步改善左室收缩功能和缩小梗塞面积。Isovolumetric hemodilution treatment reduces ischemic injury, and superoxide dismutase (SOD) may reduce reperfusion injury. However, the effects of simultaneous treatment with both interventions have never been investigated. In the present study, anesthetized rabbits underwent 45 min of coronary artery ligation, which was followed by 180 min of reflow. Ten animals in each group recieved: ①saline solution only; ②hemodilution with dextran 40 during ischemia; ③SOD before and during reperfusion; ④combined treatment with hemodilution and SOD. Left ventricular peak pressure (LVP) and +dp/dt max was improved significantly during reperfusion in hemodilution group and SOD group and myocardial systolic functions were improved further in hemodilution+SOD group as compared with that in controls. In control rabbits infarct size (percentage of the infarct size to the total left ventricle) averaged 18.1±2.3%.
