原发性肾病综合征患儿的血脂变化

摘　　要：目的:观察不同病期原发性肾病综合征(NS)的血脂变化及其变化规律。方法:对40例NS 患儿在活动期和缓解期(激素改隔日疗法时)血清总胆固酯(TC)、甘油三酯(TG),高密度脂蛋白胆周醇醋(HDL-Ch)、载脂蛋白 AI(APO-AJ)和载脂蛋白 B_(100)(APO-B_(100))进行检测。结果:活动期NS 患儿 TC(9.88±2.59)mmol/L、TG(3.06±1.72)mmol/L、APO-B_(100)(2.37±0.46)g/L,水平明显高于对照组(P<0.01),APO-AI、HDL-Ch 水平也高于对照组,但差异无显著性,缓解期 NS 患儿 TC(7.61±1.82)mmol/L、TG(2.45±1.84)mmol/L、APO-B_(100)(2.12±0.42)g/L 水平较活动期水平降低,但仍高于对照组(P<0.05)。结论:NS 患儿无论在活动期或缓解期均有明显的脂代谢异常,有必要早期应用降脂药物,以减轻肾损害和心血管并发症。Objective:The aim of this experiment was to observe the changes of serum lipid in children with idiopathic nephrotic syndrome at different stages.Methods:Serum total cholesterol(TC)triglyceride (TG),high density lipoprotein-cholesterol(HDL-ch)apolopoprotein AI(Apo-AI)and apolopoprotein B100(Apo-B100)were detenninated in 40 children with normal renal function at different stages of idopathic nephrotic syndrome(NS).Results:In the patients with active NS,TC(9.88±3.59)mmol/lTG (3.06±1.72)mmoL/l,Apo-B100(2.37±0.46)g/l level were significantly higher than that in control group(P<0.01).The concentrations of HDL-ch,and Apo-AI were increased,but whey were not significantly different form the control(P<0.5).In the patients with remission NS.TC,TG and Apo- B100 levels were decreased as compared with the active group,but these children had still increased TC (7.61±1.82)mmol/l TG(2.45±1.84)mmol/land Apo-B100(2.12±0.42)g/l levels than the control group(P<0.05).Conclusion:The above findings suggested that the children with active and remission NS had marked abnorma-lities of serum lipid early administration of reducing lipid drugs is indicated for treatment of NS to reduce the renal injuly and complicated careio-vascular diseases.
