急性非Q波型心肌梗塞心电图分型意见及其临床特点

摘　　要：目的:本文对急性非 Q 波型心肌塞(ANQMI)的心电图(ECG)分型与临床特点进行比较。方法:根据 ECG 改变,ANQMI 分成 ST 段压低型、T 波倒置型和复合型。ST 段压低型与复合型为 A组,T 波倒置型为 B 组,两组就性别、年龄、MI 前主要病史、MI 后主要并发症及病死率进行比较。结果:①A组男6例(32%),女13例(68%),平均年龄63.9±9.6;B 组男27例(71%),女11例(29%),平均年龄59.1±8.8;②A组 MI 前与 MI 后心绞痛发生率分别为74%与84%,B 组分别为59%与51%;③A组与 B 组,急性 QMI 发生率分别为26%与2.6%(P<0.05),泵衰竭分别为37%与21%,住院死亡率分别为21%与10.5%(P<0.05)。结论:A 组与 B 组比较,其临床特点是:①老年女性发病率高;②有 MI 前心绞痛的患者,MI 后仍可有心绞痛发作;③再梗塞。Objective:The article compared electrocardiogram(ECG)types with clinical characters of acute Non-Q wave myocardial infarction(ANQMI).Methods:ANQMI of 57 cases diagnosed depending on ST-T changing of ECG were divided into three types:ST segment depressed type.T wave reversed type and compound type.ST segment depresed type and compound type were put in group A,T wave re- versed type put in group B.Two groups were compared at sex ,age,chief disease history before myocardial infarction(MI),chief complication after MI and mortality.Results:①in group A,male 6(32%),female 13(68%),mean age 63.9±9.6;in group B,male 27(71%),female 11(29%),mean age 59.1±8.8; ②in group A,inci-dences of fore infarction angina and post infarction angina were 74% and 84% re- spectively;in group B were 59% and 51%;③in group A and B,incidences of acute Q wave MI were 26% and 2.6% respectively(P<0.05);incidences of pump failture were 37% and 21% respectively; mortalitys in hospital were 21% and 10.5% respectively(P<0.05);Cuaclusion:When compared with group B,clinical characters in group A were following;①incidence of older female was higher;②patients with fore infarction angina still might be subject to angina attack after MI;③incidenee of reinfaretion, pump failture and mortality were higher.
