儿童哮喘血清可溶性白细胞介素-2受体研究

摘  要:目的:观察儿童哮喘不同病期血清可溶性白细胞介素-2受体(sIL-2R)水平。方法:采用双抗体夹心 ELISA 法对26例儿童哮喘急性发作(发作组)、16例哮喘急性发作中给予糖皮质激素治疗(发作+激素治疗组)、20例儿童哮喘临床控制(缓解组)和20名健康儿童的血清 sIL-2R 水平进行测定。结果:发作组血清 sIL-2R 水平(539.76±143.82U/ml)显著高于缓解组(297.21±72.16U/ml,P<0.01)和对照组(246.18±63.24U/ml,P<0.01),发作+激素治疗组血清 sIL-2R 水平(386.42±97.40/ml)较治疗前显著降低(P<0.05),但仍明显高于缓解组(P<0.05),缓解组血清 sIL-2R 与对照组比仍在较高水平(P<0.05);发作组血清 sIL-2R 水平与 PaO_2呈显著负相关(r=-0.7215,P<0.01),发作+激素治疗组中各例血清 sIL-2R 水平较治疗前的变化值与 PaO_2变化值亦呈显著负相关(r=-0.4316,P<0.05)。结论:sIL-2R 可能涉及儿童哮喘发作的病理生理过程。Objective:This study is to observe the serum level of soluble interleukin-2 receptor(sIL- 2R)of asthmatic children in different period of the disease.Methods:The serum sIL-2R has been determined by sandwich ELISA method with two monoclonal antibodies in 26 cases of asthmatic children during attack(attack group),16 cases of asthmatic children during attack after 3 day oral administration of corticosteroids(attack-and-steroid group),20 cases of non-symptomatic asthmatic children(remission group)20 normal volunteers(control group).Results:The serum sIL-2R in attack group(539.76± 143.82)U/ml was much higher than that in both remission group(297.21±72.16)U/ml,P<0.01 and control group(246.18±63.24 U/ml,P<0.01).The serum sIL-2R in attack-and-steroid group (386.42±97.40 U/ml)was lower than that in attack group(P<0.05) ,but was still higher than that in control group.The serum sIL-2R in attack group revealed notably negative correlation to PaO_2(r=- 0.7215,P<0.01),and the changes of the serum sIL-2R after oral administration also revealed notably negative correlation to the changes of PaO_2(r=-0.4316,P<0.05).Conclusion:The sIL-2R probably involved in oathophysiologic process of acute attack of asthmatic children.
