摘　　要：目的:探讨规范三级康复治疗对缺血性脑卒中患者功能独立性的影响。方法:将164例缺血性脑卒中患者随机分为康复组和对照组,分别在入组时、发病后1个月、3个月和6个月对患者进行功能综合量表(FCA)评分,并进行统计学分析。结果:对照组与康复组在入组时FCA总得分、运动功能评分和认知功能评分在统计学上均未见显著性差异。对照组的FCA总分从发病后1个月、3个月到6个月逐渐增高,但只有1个月和3个月比较时有显著性差异;康复组亦呈现出逐渐恢复的趋势,但其恢复程度明显高于对照组,且各次评定之间比较均有显著性差异。对照组FCA运动功能评分只有1个月和3个月比较时有显著性差异,而康复组各次评定之间比较均有显著性差异,且恢复程度明显大于对照组。对照组FCA认知功能评分无明显增加,各次评定之间比较也未见显著性差异;康复组认知功能评分在1个月、3个月和6个月时均明显好于对照组,且呈现逐渐增加趋势,但只有1个月与入组时比较有显著性差异。结果表明:规范的三级康复治疗对提高缺血性脑卒中患者功能独立性具有重要作用。Objective In order to explore the effects of standardized three--stage rehabilitation program on the functional independence in patients with ischemic stroke. Methods 164 patients with ische- mic stroke were randomly recruited into rehabilitative and control groups. The function comprehen- sive assessment （FCA） was performed at the recruitment （M0）, the end of the 1st （M1）, 3rd （M3） and 6th （M6） month after stroke. Results No significant differences were found in the total FCA, motor FCA and cognitive FCA between the rehabilitative and the control groups. The total FCA of the control group increased gradually at the recruitment, the end of the 1st, 3rd and 6th month, but the significant differences were found only at the end of the 1st month and 3rd month comparing with previous assessments. The total FCA increments in rehabilitative group were more significant than that in the control group at the end of the 1st, 3rd and 6th month, and significant differences were found between neighboring evaluations in rehabilitative group. The motor FCA in the control group revealed significant differences only at the end of the 1st and 3rd month comparing with previous assessment. The increments of the motor FCA in the rehabilitative group were more significant than that in the control, and there were significant differences between the neighboring evaluations in rehabilitative group. The cognitive FCA of the control group increased insignificantly, and no significant differenceswere found between the neighboring assessments. The cognitive FCA in the rehabilitative group re- vealed improving tendency and the increments were more significant than that in control group at the 1st, 3rd and 6th month, but significant difference was only found at the 1st month comparing with that at the recruitment in the rehabilitative group. The study indicated that the standardized three-stage rehabilitation program could enhance the functional independence in patients with ischemic stroke.
