摘 要:[目的]研制一种新型复方黄连氧气湿化液,以解决现行氧疗中湿化液污染问题。[方法]采用二阶段交叉设计方法,比较使用不同时间段蒸馏水湿化液与复方黄连氧气湿化液的细菌培养结果及合格率;确定复方黄连氧气湿化液的更换时间。同时向氧疗病人发放问卷,调查氧疗时的气味、病人氧疗后的舒适度及呼吸道不适感。[结果]使用24h的氧气湿化液采样培养:复方黄连液菌落数低于蒸馏水,差异有统计学意义(P<0.05);采样合格率高于蒸馏水,差异有统计学意义(P<0.05);使用复方黄连液的病人吸氧时舒适感高于使用蒸馏水的病人,而吸氧后咽部干燥不适感低于使用蒸馏水的病人,差异均有统计学意义(P<0.05)。依据使用24h、48h复方黄连氧气湿化液的采样结果,确定复方黄连氧气湿化液的更换时间为48h。[结论]复方黄连液可有效降低湿化液的污染,减少医院内肺部感染的发生,提高氧疗舒适度及病人依从性,可作为一种新型的氧气湿化液在临床做进一步的探讨。Objective:To manufacture a new type of compound golden thread oxygen humidifying fluid so as to solve the problem of humidifying fluid contamination during oxygen therapy at present.Methods:Both the result of bacterial culture and qualified rate of different time periods of distilled water humidifying fluid(distilled water group) and compound golden thread oxygen humidifying fluid(compound golden thread group) were compared by adopting two-stage cross-over design method.And then the changing time of compound golden thread oxygen humidifying fluid was determined.At the same time the questionnaires were distributed for patients undergoing oxygen therapy.The smell during oxygen therapy,comfort degree and uncomfortable feeling of respiratory tract of patients after oxygen therapy were investigated.Results:Both colony counts of samplings and throat dryness and uncomfortable of patients in compound golden thread group were lower than that of those in distilled water group after 24 hours of using oxygen humidifying fluid.However,both the qualification rate of sampling and the comfortable level of patients in compound golden thread group were higher than that of those in distilled water group.There were statistical differences between the two groups(all P〈0.05).The time of replacing compound golden thread fluid was determined at 48 hours based on the outcomes of sampling from distilled water as humidifying fluid and compound golden thread humidifying fluid of different using time periods.Conclusion:Compound golden thread fluid can reduce the contamination of humidifying fluid and decrease the iatrogenic pulmonary infection.It can enhance both the comfort level and the compliance of patients undergoing oxygen therapy.However,as a new type of oxygen humidifying fluid,its safety,effectiveness and economics are need to be probed into further in clinical practice.
