系统性红斑狼疮患者外周血调节性T细胞、Foxp3和IL-6的表达

摘  要:目的探讨系统性红斑狼疮(systemic lupus erythematosus,SLE)患者外周血CD4+CD25+调节性T细胞(regulatory Tcells,Tregs)、Foxp3mRNA和血浆IL-6表达的意义。方法对38例SLE患者和16例正常人采用流式细胞术检测外周血CD4+CD25+Tregs百分率,RT-PCR检测Foxp3mRNA表达,ELISA法检测IL-6水平。结果①SLE活动组和非活动组的CD4+CD25+Tregs水平均显著低于正常对照组(P<0.01);②SLE活动组的Foxp3mRNA表达水平明显低于正常对照组(P<0.05);③SLE活动组和非活动组血浆的IL-6水平均显著高于正常对照组(P<0.01),而且活动组显著高于非活动组(P<0.05);④38例SLE患者的CD4+CD25+Tregs水平与SLEDAI评分呈显著性负相关(P<0.01),Foxp3mRNA水平与CD4+CD25+Tregs呈显著性正相关(P<0.01),血浆IL-6水平与SLEDAI评分之间呈显著性正相关(P<0.01),血浆IL-6水平与CD4+CD25+Tregs/CD4+细胞比值呈显著负相关(P<0.05)。结论CD4+CD25+Tregs和Foxp3以及IL-6可能在SLE的发生和发展中发挥重要作用。Objective To explore the implication of the expression of CD4^+ CD25^+ regulatory T cells (Tregs), Foxp3 mRNA in peripheral blood and plasma IL-6 in the patients with systimic lupus erythematosus (SLE) . Methods CD4^+ CD25^+ Tregs were detected by flow cytometry, Foxp3 mRNA was assayed by polymerase chain reaction and the plasma IL-6 was measured by ELISA from 38 SLE patients and 16 normal controls. Results (1) The expressions of CD4^+ CD25^+ Tregs in both the active and inactive SLE patients were lower than that of normal controls ( P 〈 0. 01 ) ; (2) The expression of Foxp3 mRNA in the active SLE patients was significantly lower than the normal controis ( P 〈 0. 05 ) . There was no significant difference between active and inactive groups ; (3) The plasma level of IL-6 was significantly increased in both the inactive and active SLE groups as compared with normal control group (P 〈0. 01, P 〈0. 01, respectively), while the plasma IL-6 in the active SLE group was significantly higher than in the inactive SLE group (P 〈 0. 05 ) ; (3) The expression of CD4^+ CD25^+ Tregs in 38 SLE patients was negatively correlated with SLEDAI scores ( r = -0. 81, P 〈0. 01 ), the expression of Foxp3 mRNA was positively correlated with CD4^+ CD25^+ Tregs (r =0. 78, P 〈 0.01 ) and the corresponding plasma level of IL-6 was positively correlated with SLEDAI scores ( r = 0. 58, P 〈 0. 01 ) . In addition, there was a negative correlation between the IL-6 level and CD4^+ CD25^+ Tregs / CD4^+ T cells ( r = - 0. 389, P 〈 0. 05 ). Conclusion The expressions of CD4^+ CD25^+ Tregs , Foxp3 mRNA in peripheral blood and IL-6 in plasma might play important roles in the pathogenesis of SLE.
