乙型肝炎病毒基因分型的临床探讨

摘　　要：目的探讨乙型肝炎病毒(HBV)基因分型与临床的关系。方法采用HBVS区直接测序法对我院164例江苏、安徽籍患者进行基因分型。结果164例患者中C型97例,B型63例,BC混合型4例。HBeAg阳性116例,B型38/63例(60.31%),C型76/97例(78.35%),HBV DNA≥105124例,B型43/63例(68.25%),C型81/97例(83.51%)。差异有统计学意义(P<0.01)。6个家庭的13例HBV感染者进行HBV分型检测,子女的HBV基因型与母亲相同。14例慢性乙型肝炎患者采用苦参素治疗,以B型效果明显。结论江苏、安徽地区的HBV基因分型以C型为主,其次为B、BC混合型,未见A、D、E、F、G、H基因型。C基因型HBeAg阳性多,HBV DNA定量值高。子女的HBV基因型与其母亲相同。Objective To explore the relationship between HBV genotype and curative effect of kushenin. Methods Blood specimens of 164 HBsAg - positive patients coming from Jiangsu Province or Anhui Province were detected for HBV genotype by determining nucleotide sequences in the HBVS gene directly. Results In these cases, 97 of them were HBV genotype C, 63 patients were HBV genotype B and 4 patients were HBV genotype BC. Among 116 HBsAg/HBeAg - positive patients, 38/63 （60.31%） patients were HBV genotype B, 76/97（78.35% ）patients were HBV genotype C. Of 124 patients who with a viral load ≥ 10^5copies/ml, 43/63（68.25% ） patients were HBV genotype B, 81/97（83.51% ） patients were HBV genotype C, and there were statistical significance （ P 〈0. 01 ）. The analysis of HBV genotype of 13 patients coming from 6 families indicated that children＇ s HBV genotype were all the same with their mothers. The follow - up study of administrating kushenin to 14 patients suggested that the therapeutic efficacy of HBV genotype B after kushenin treatment is better than other HBV genotype. Conclusion HBV genotype C are the predominant HBV genotype in Jiangsu province or Anhui province, then are HBV genotype B or HBV genotype BC, while HBV genotype A, D, E, F, G, or H is not detectable in any patient. The rate of HBeAg positivety and the level of HBV - DNA of HBV genotype C are significantly higher than other HBV genotype. Children＇ s HBV genotypes were all the same with their mothers.
