中西医结合治疗舒张功能不全心力衰竭60例对照观察

摘　　要：目的:探讨参乌合剂对舒张功能不全心力衰竭的治疗效果。方法:选符合纳入标准的舒张功能不全心力衰竭患者120例,随机分为治疗组、对照组各60例。治疗组在西医常规治疗基础上,予参乌冠心冲剂;对照组仅予西医常规治疗。疗程1个月。结果:治疗组显效35例,有效20例,无效5例,总有效率为91.67%(95%CI=84.68%～98.66%);对照组显效26例,有效18例,无效16例,总有效率为73.33%(95%CI=62.14%～84.52%);两组综合疗效比较,有显著性差异(u=2.0283,P=0.0427);同时,治疗组治疗后症状积分、超声心动图A峰值较对照组显著下降(P<0.05);超声心动图E/A值明显升高(P<0.01)。结论:参乌合剂合西医常规治疗舒张功能不全心力衰竭较单纯西医常规治疗的疗效为优,其收益为OR=0.25(95%CI=0.08～0.74),NNT=5(95%CI=3.13～21.10)。并可改善舒张末期心肌的顺应性,改善心功能。Objective： To observe the curative effect of Granule of Ginseng and Monkshood on heart failure of diastolic functional defect. Method： 120 cases of heart failure of diastolic functional defect were selected and divided into the treatment group and the control group randomly. And each group includes 60 cases. The cases in the treatment group were treated with Granule of Ginseng and Monkshood combining with western medicine, and the ones in the control group were treated only with western medicine. The course of treatment is 1 month. Result： In the treatment group 35 cases were excellent, 20 cases were effective and 5 cases were ineffective. The total effective rate was 91.67% （95 %CI=84.68%-98.66%）. In the control group 26 cases were excellent, 18 cases were effective and 16 cases were ineffective. The total effective rate was 73.33 % （95 %CI= 62.14 % -84.52 %）. The difference between the two groups is significant（u=2. 028 3,P=0. 042 7）. At the same time the symptom integrate and A crest value in echocardio- gram were more descent significantly in the treatment group than in the control group （P〈0.05）, and E/ A value in echocardiogram was ascendant obviously （P〈0. 01）. Conclusion： The effect of Granule of Ginseng and Monkshood combining with western medicine on heart failure of diastolic functional defect is better than only with western medicine. OR = 0.25 （95 % CI = 0.08- 0.74）, NNT ： 5 （95 % CI = 3.13 21.10）. And in the treatment group the myocardial complaisance in end-diastole was improved and the heart function was ameliorated.
