小儿先天性皮毛窦的外科治疗(附7例报告)

摘　　要：目的:总结小儿先天性皮毛窦的诊治经验提高认识及临床处理能力。方法:回顾分析2002年9月以来手术治疗和病理证实的7例先天性皮毛窦患儿资料。1例位于胸段合并脊髓纵裂,6例位于腰骶部,其中3例合并脊髓栓系,1例皮样囊肿,1例表皮样囊肿,1例脂肪瘤。有2例患儿反复发生化脓性脑膜炎。术前均行MRI检查明确诊断。手术切除窦道同时一并切除合并的囊肿及脂肪瘤,切除脊髓纵裂的骨嵴,切断终丝松解圆锥。结果:完全切除6例,1例残留部分囊壁以碘酒烧灼。术后1例出现脑脊液漏经保守治愈,1例出现暂时性尿失禁。结论:先天性皮毛窦临床表现多样,部分病人没有任何症状,MRI检查是首选的诊断手段,早期诊断及显微外科手术治疗是最佳选择。Objective：To review the characteristics and therapeutic outcomes of congenital spinal dermal sinus in children. Methods- The clinical data of 7 cases with congenital spinal dermal sinus from september 2002 to right now were retrospectively analyzed. Results：Completely removal of the cysts were achieved in 6 cases while part cystic lesions were residual in another 1 case. Conclusion： The features of congenital spinal dermal sinus varied in clinical cases and MILl was the best diagnostic method for such patients.Early diagnosis and microsurgical operation should be the best therapeutic measurement.
