摘 要:目的研究异丙酚对大鼠急性脊髓损伤的影响。方法雌性SD大鼠60只,体重230~270 g,随机分为3组(n=20):假手术组、脊髓损伤组和异丙酚组。采用改良的Allen打击法致伤大鼠脊髓,打击后30 min,异丙酚组经尾静脉持续输注1%异丙酚60 mg·kg^-1·h^-1 h,其余2组持续输注0.9%生理盐水6 ml·kg^-1·h^-1 1 h。应用斜板实验评分法和BBB联合评分法评价后肢运动功能;采用原位末端标记法(TUNEL法)检测脊髓组织细胞凋亡;HE染色后光镜下观察脊髓组织病理学。结果异丙酚组行为学评分高于脊髓损伤组(P〈0.05);与假手术组相比,脊髓损伤组各时点单位面积凋亡细胞数均升高(P〈0.01);与脊髓损伤组相比,异丙酚组各时点单位面积凋亡细胞数均降低(P〈0.05);脊髓损伤组脊髓病理损伤较异丙酚组重,可见大量神经元坏死。结论持续静脉输注1%异丙酚60 mg·kg^-1·h^-1 1h可减轻大鼠急性脊髓损伤,其机制与抑制细胞凋亡有关。Objective To investigate the effect of propofol on acute spinal cord injury and the possible underlying mechanism. Methods Sixty female SD rats weighing 230-270 g were randomly divided into 3 groups ( n = 20 each) : Ⅰ sham operation group (S) ; Ⅱ injury group (Ⅰ) and m prepofol group (P). Acute spinal injury was produced by spinal cord contusion according to Allen's method. The animals were anesthetized with intraperitoneal 20% urethane 1.0 g/kg. A median incision was made in the back. The spinous process of T8 vertebra was used as the mid point of the incision. The spinal cord was exposed. A weight of 10 g was dropped freely at the height of 4 cm. In prepofol group 1% propofol was infused at a rate 60 mg · kg^- 1· h^- 1 for an hour starting from 30 min after trauma. In group Ⅰ and Ⅱ equal volume of normal saline was infused instead of prepofol. The inclined plane and BBB (Basso, Beattie and Bresnahan) scale were used to assess hind limb neurological function. The apoptosis was measured by using TUNEL technique. Histo-pathological changes were examined. Results Behavioral score was significantly higher in prepofol group than in injury group (P 〈 0.05 ). The apoptotic rate was significantly increased in injury group as compared with sham operation group. Propofol infusion significantly decreased the higher apoptotic rate induced by spinal injury. A large number of dead neurons could be seen in injury group. Propofol infusion significantly ameliorated the pathological changes induced by spinal injury. Conclusion Propofol infusion can ameliorate acute spinal injury in rats by decreasing apoptosis.
