钩端螺旋体病神经系统后发症患者L型检测及其意义

摘　　要：为探讨钩端螺旋体病神经系统后发症与钩体Ｌ型密切关系，本文采用显微镜凝集试验（ＭＡＴ）、钩体普通培养和Ｌ型培养方法对１８６例钩体病神经系统后发症患者共２０６份标本（血液１３５份，脑脊液７１份）进行了检测。结果：血液Ｌ型培养阳性率（２７．４％）明显高于血液ＭＡＴ阳性率（１７．０％）（０．０２＜Ｐ＜０．０５），脑脊液Ｌ型培养阳性率（３０．９％）显著高于脑脊液ＭＡＴ阳性率（１２．７％）（Ｐ＜０．０１），钩体Ｌ型培养总阳性检出率（２８．６％）显著高于钩体普通培养阳性检出率（１０．２％）（Ｐ＜０．００１）。结果表明，用于钩体病神经系统后发症患者血液和脑脊液标本检查时，Ｌ型培养法优于ＭＡＴ和普通培养法。To investigate the late complication in lnervous system related with leptospiral L-form. 206 specimens (135 blood and 71 CSF) derived from patients with late complication in nervous system were detected by using microscopic agglutination test (MAT)and isolation of leptospira and their L-forms.The results showed that positive rate of leptospiral L-form in blood,27 4% and CSF,30 9% was higher than that of MAT in blood,17 0% and CSF,12 7%(0 02<P<0 05 and P<0 01). The total isolation rate for leptospiral L-form(28 6%) was also much higher than that of leptospira(10 2%) in both blood and CSF(P<0 001).The results indicated that isolation for leptospiral L-form was very helpful in diagnosis of late complication in nervous system.
