摘　　要：目的分析深圳市居民就医流向对“看病难,看病贵”问题的影响及其程度,为卫生政策的制定提供科学依据。方法采用随机问卷调查,了解深圳市居民的门诊就医流向、就诊时间成本和费用负担,以及居民对“看病难、看病贵”的感受,分析就医流向与“看病难,看病贵”问题的关系。结果调查对象两周内患病以常见病和多发病为主,但有28.6%的患者门诊就医在区级/市级医院,16.7%在街道医院,社康中心就诊的比例为37.9%,低级别医疗机构,尤其是社康中心卫生资源闲置。患者在区/市级医院的门诊就诊费用和住院费用高于其他级别医疗机构。结论深圳市居民就医流向过于向高级别医疗机构集中,加剧了卫生资源的紧张和医疗费用的不合理增长。Objective To provide scientific evidences for formulating social health policy by analyzing the influence of the out-patients flow on the problem of difficulty and high cost of getting medical treatment and the extent of influence. Methods By method of random questionnaire survey, we collected the information of the flow of out-patients, the time cost of medical treatment and the burden of medical expenses in different medical institutions, and the feelings of residents in getting medical treatment at all levels of medical institutions in Shenzhen. Then the relationship between the flow of out-patients and the problem of difficulty and high cost of getting medical treatment was analyzed. Results The investigated residents went to see the doctor mainly for the common diseases and frequently occurred diseases within two weeks. Totally, 28. 6% of patients got medical treatment in out-patients of the district and municipal hospitals, 16. 7% of patients in the street hospitals, and 37. 9% of patients in the community health centers. The health resources were idle in low-level medical institutions, especially in community health centers. The expenses of medical treatment at out-patients and in-patients in the district and municipal hospitals were higher than those in other medical institutions. Conclusion The flow of the out-patients in Shenzhen is over concentrated in high-level medical institutions, thus exacerbates the insufficiency of health sources and unreasonable increase of medical expenses.
