摘  要:目的了解浙江省武义县某敬老院发生戊型病毒性肝炎(戊肝)疫情的原因,并采取控制措施。方法开展问卷调查和病例对照研究。用SPSS13.0进行数据分析;采集敬老院所有人员88份及相关密切接触者113份血清、猪血清7份、鼠血清4份,用酶联免疫吸附试验检测戊肝抗体(抗-HEV)。结果18份血清标本抗-HEV阳性,猪、鼠血清抗-HEVIgM检测均为阴性。是否喝生水、洗碗后是否用开水冲洗等因素的调查结果,有显著或非常显著的统计学意义[X^2=5.37,Fisher P=0.033,OR=12.4,95%可信区间(CI)1.20~127.75;X^2=14.94,P=0.000,OR=0.084,95%CI0.02~0.35]。临床表现以茶尿、厌食为主,无发热。结论敬老院的戊肝爆发与饮用水污染有关,基本排除食物因素,老年人戊肝临床表现较轻。Objective An outbreak of HEV occurred in a nursing home on May 9,2006 in Wuyi County,Zhejiang Province. To make clear the cause of the outbreak, we conducted an investigation,taking control measures timely. Methods The investigation was made by face-to-face questionnaire and conducted a case control study. The data were analyzed by SPSS 13.0. We col lected 88 sera from all the members of the nursing home and 113 sera from the close contacts and screened subjects,7 sera from pigs,4 from rats and detected antibody of HEV by ELISA. Results Of 201 serum samples, 18 detection results of HEV antibody are positive. Anti-HEV. IgM of the sera from pigs and rats are negative. The results of the risk factors, such as drinking raw water and washing bowl by boiled water, were as following : X^2= 5. 374, Fisher P = 0. 033, OR : 12.4,95%CI(1.2-127.7);X^2= 14.94,P=0. 000,OR=0. 084(0. 020-0. 35),respectively. Most of the symptom of cases were tawny urine, anorexia, and xanthochromia but no fever. Conclusions The outbreak of HEV are concerned on contaminated water and not food-borne. The symptoms of most of cases were mild.
