摘  要:目的:研究新疆哈萨克族人群体质指数(BMI)、血脂水平与β2-肾上腺素能受体+491C/T多态性的关系。方法:以人群为基础的病例-对照研究,针对528例30~60岁哈萨克族人,测量其身高、体重,在此基础上计算BMI,按照WHO的标准将其划分为正常(18.5≤BMI<25)、超重(25≤BMI<30)及肥胖(BMI≥30)3组,并采集其血液标本,测定TC、TG、HDL-C,计算LDL-C值,同时采用饱和酚/氯仿法提取外周血白细胞基因组DNA,应用PCR-RFLP技术检测β2-肾上腺素能受体+491C/T基因型及等位基因频率;采用方差分析、t检验、Fisher’s精确概率检验、Logistic回归等统计学分析方法,观察不同BMI人群血脂水平的变化、不同基因型人群BMI和血脂水平差异、β2-肾上腺素能受体+491C/T基因型及等位基因频率的分布和TG、TC、BMI与β2-AR基因+491C/T多态性的关系。结果:正常、超重、肥胖3组间血清TC、TG的水平随BMI增加而增高(均P<0.01)。正常组β2-肾上腺素能受体+491C/T基因型频率分别是CC98.91%、CT1.09%,超重加肥胖组为CC98.8%、CT1.2%;正常组等位基因频率分别为C99.45%、T0.55%,超重加肥胖组C99.4%、T0.60%,分布的差异均无统计学意义。各基因型间BMI、TC、TG差异无统计学意义。结论:β2-AR基因+491位点C/T多态性可能不是新疆哈萨克族人群BMI、TG、TC异常的易感因素,而TC、TG水平增高是该人群超重、肥胖的危险因素。Objective:To study the relationship among different body mass index(BMI) ,lipid levels and the + 491C/T polymorphism of 132-adrenegic recepto gene(132-AR gene) in Kazakns of Xinjiang. Method: In this population-based case-control study,528 Kazakns subjects (30--60 years old) from the pastoral areas in Xinjiang were recruited and divided into three groups according to the standard of WHO about the measurement of normal, overweight and obesity(18. 5≤BMI〈25 and 25≤BMI〈30 and BMI≥30 respectively). Peripheral blood were collected from all recruited subjects. The triglyceride (TG), total cholesterol (TC), high density lipoprotein-cholesterol (HDL-C) and low density lipoprotein-cholesterol(LDL-C) levels were measured. DNA was extracted by means of a standard phenol-chloroform method. β2-AR genotype was identified by PCR-RFLP in all the subjects. Then the lipid levels in different BMI groups, the BMI and lipid levels between two genotypes, the distribution of genotypic and allelic frequencies among three groups were studied. Result: In this study, the TG, TC levels had significant difference among three groups classified according to BMI, and with the increasing of BMI, the TG and TC levels were raised(P= 0. 004,0. 000 respectively). In this population, the frequencies of the genotype CC, CT were 98.86% and 1.14% respectively,the allelic frequencies of C,T were 99.43% and 0.57%. The genotypic frequencies and allelic frequencies were not significantly different among three groups(P〉0. 05). BMI, TG TC had no significant difference in the groups classified according to genotypes ( P〉 0.05 ). Conclusion : +491C/T polymorphism of β2-AR gene may not be associated with abnormal BMI , TG , TC in Kazakns of Xinjiang, which implicates that it may not play an major role in the mechanism of abnormal BMI ,TG ,TC in Kazakns. Higher levels of TC, TG are risk factors for overweight and obesity in Kazakns.
