昂丹司琼复合杜冷丁对抗内脏牵拉反应的临床观察

摘　　要：目的观察硬膜外麻醉下昂丹司琼复合杜冷丁对抗内脏牵拉反应的效果及其对呼吸循环的影响。方法180例ASAⅠ～Ⅱ级择期行上腹部手术的患者,随机分成A、B、C 3组。A组为昂丹司琼-杜冷丁组60例、B组为杜氟组60例、C组为对照组60例。手术开始前5～10 min静脉注入辅助药。A组缓慢静注昂丹司琼0.16 mg/kg、杜冷丁1.0 mg/kg;B组静注氟哌啶0.1 mg/kg、杜冷丁1.0 mg/kg;C组无辅助用药。分别监测BP、HR、SpO2;观察探查及牵拉内脏器官时患者的反应。结果A组用药后51、0 min的MAP、HR、SpO2与用药前比较无统计学意义（P〉0.05）。A、B两组对抗内脏牵拉反应的效果明显好于C组（P〈0.01）,A组效果好于B组（P〈0.01）。结论硬膜外麻醉下昂丹司琼复合杜冷丁对抗内脏牵拉反应效果确切,对呼吸、循环无明显影响。Objective To observe the effeetion of viscera retracting reaction respiratory and eireulation system with dolantine and ondansetron under epidural anesthesia. Methods 180 eases of ASA Ⅰ～Ⅱ degree upper abdominal op- eration patient were divided into 3 groups 60 eases per each. Group A was treated with ondansetron 0.16 mg/kg and dolantine 1 mg/kg. Group B was given dolantine 1 mg/kg and droperidol 0.1 mg/kg and group C was control group. The Bp, HR, SpO2 and retracting reaction were observed. Results The data of MAP, HR and SpO2 had no difference between group A after 5 rain and 10min. The effect in group A was better than that in group B. The retracting reaction in group A and B was better than that in group C. Conclusion There might have good effect on retracting reaction and no effeetion on respiratory and circulation system with this method.
