肝静脉内支架成形术后的彩色多普勒超声研究

摘　　要：8例三支肝静脉阻塞的布-加氏综合征患者，在超声引导下行肝静脉内支架成形术，术后即刻及术后一周应用彩色多普勒超声观察肝脏血流动力学的变化。结果表明肝静脉置入支架并恢复通畅后，彩色多普勒血流显像（CDFI）及血流频谱显示肝内血液循环途径发生明显改变，肝静脉内径由术前12．6士3．2mm，术后即刻降为7．8士1．7mm（P＜0．05），术后一周为7．6士1．8mm（P＜0．05）；峰值流速由术前0．7士0．3cm／s，术后即刻升至2．3士1．1cm／s（p＜0．01），术后一周稍回降至1．5士0．9cm／s（p＜0．05）。其中一例支架术后一周闭塞，CDFI示其内无血流通过。表明肝静脉内支架术后肝脏血流动力学的改变复杂而明显，CDFI能定性定量检测血流动力学，明确支架通畅与否，可作为主要随诊手段之一。Eight patients with Budd-Chiari syndrome complicated by three main hepatic veins (HVs) obstruction were studied by color Deppler flow imaging (CDFI) after hepatic vein stent implantations. When obstructed HVs were recanaled and stented, CDFI and pulsed Deppler showed marked changes of hepatic hemodynamics. The diameters of the stented HV were reduced from 12. 6±3. 2mm to 7. 8±1. 7mm instantly after oPerati0n (p＜0. 05 ) and to 7. 6±1. 8mm one Wee after operation (p<0. 05). The velocities of HVs were increased from 0. 7f0. 3cm/s to 2.3±1. 1cm/s instantly after oPeratbo (p<0. 01) and to 1. 5±0. 9cm/s one week after operation (p<0. 05). One stent was re-occluded after one week and CDFI showed no blood flow in it. In conclusion, the CDFI technique can give qualitative and quantitative studies of the instan hepotic hemodynamic changes, and clarify the patency and the long term effects of the stent.
