动脉导管未闭介入治疗的疗效分析

摘　　要：目的探讨国产封堵器介入治疗动脉导管未闭的临床疗效及随访结果。方法全组病例93例,年龄2-46岁,体重7-65kg,男42例,女51例,PDA直径1.0-19mm,肺动脉压力18-110mmHg,PDA封堵器的大小4-30mm,术前B超筛查,术中术后测肺动脉压力,主动脉弓处造影,均选用国产封堵器。结果1例因PDA〈1mm而放弃,92例成功,根据造影结果结合B超数据选择合适大小封堵器,即刻造影部分有分流并闻及杂音,15min造影1例仍有少量分流并能听到杂音,但1天后杂音消失。57例经半年以上随访,56例无其他任何并发症,1例术后1月发生贫血现象。结论介入治疗PDA安全有效,是单纯PDA的首选方法。Objective To evaluate the curative efficacy of non-surgical closure of patent ductus arteriosus （PDA） with Chinese - made septal occluders. Methods This series consisted of 93 patients, aged 2 - 46 years, weighting 7 - 65 kg,42 males and 51 females, with patent ductus arteriosus 1.0 - 19 mini （6.2 ± 3.03）mm] in diameter and pulmonary artery pressure 18-110 mmHg. B- mode ultrasound examination before operation and aortic arch tomography during and after operation helped choos proper septal occluders, ranging from 4 mm to 30 mm[（ 10± 2）mm）. Results One patient quitted whose PDA was only 1 mm in diameter,and others were success.Shunt in tomography was seen and souffle was heard immediately after the procedure, which disappeared after 15 min,in some patients,and lasted till the next day ony in one pationt. 57 cases were followed up for more than half a year and no complications occurred in 56 of them. Anemia developed in 1 case one month after operation. Conclusions Chinese - made septal occluders are safe and effective for closure of PDA.
