肌电图对耻骨直肠肌综合征诊断价值的评估

摘　　要：目的:探讨肌电图对耻骨直肠肌综合征患者的诊断价值。方法:根据临床指诊和排粪造影将55名便秘患者和15名正常自愿者分为耻骨直肠肌异常组、耻骨直肠肌正常组和正常对照组,分析三组的肌电图结果。结果:①肌电图定量Mup分析:正常对照组、耻骨直肠肌正常组在静息相和力排相的各项指标无显著性差异;耻骨直肠肌异常组力排相的Ampl、Area和Freq明显高于本组的静息相,并且该组静息相的Ampl明显高于正常对照组和耻骨直肠肌正常组,Ar/Am明显低于另两组﹙P<0.05﹚;②肌电图干扰相分析:耻骨直肠肌异常组力排相的Turns、Ampl、T/A、Activity、Envelop仍明显高于本组的静息相,并且静息相的Turns、Ampl、T/A、Activity、Envelop明显高于另两组﹙P<0.05﹚。结论:通过对便秘患者的进行肌电图检测发现耻骨直肠肌的异常肌电活动是耻骨直肠肌综合征的主要原因;因此肌电图,尤其是干扰相分析,能准确可以评价盆底肌的功能状态,帮助更全面的诊断和指导临床治疗。Objective： Assessment about the diagnostic value of Electromyogram（EMG） on puborectalis syndroma. Methods： We divided 55 constipation patients and 15 normal volunteers into the puborectalis normal group, the puborectalis abnormal group and the normoal control group by the clinical symptoms,the rectal touch and the results of defecography. Then we analyzed the EMG results of three groups. Results： （1）Quantitate Mup analysis of EMG：All the indexes of the normal control group and the puborectalis normal group have not significant difference during resting and defecating phases;The indexes （including Ampl, Area and Freq） of abnormal group are much higher in defecating phase than in resting phase,and the Ampls of abnormal group during resting phase are also evidently higher than which of the control group and the puborectalis normal group;Yet the ratios Ar to Am are much lower than which of the other groups（P〈0.05）. （2）Analysis of interfered phase （IP）：In abnormal group, the indexes （including Turn,Ampl,T/A,Activity and Envelop） in defecating phases are much higher than which in resting phases,and these indexes in resting phases are significantly higher than those in the normal control and puborectails normal groups（P〈0.05）. Conclusion：Through the examination of electromyogra of the constipation patients we found that dysfunction of myoelectric activity of puborectal muscle is the cause of puborectalis syndrome. For this reason,electromyogra, especially the analysis of interfered phase,can estimate the functional ststus of pelvis-base muscles of the constipation patients, which can be also used for assisting the diagnoses and theraphy.
