中心静脉导管导丝支撑引导钢丝螺纹管插管在困难气管插管中的应用

摘　　要：目的探讨中心静脉导管导丝支撑引导行钢丝螺纹管气管内插管在困难气管插管时应用的有效性和安全性。方法将80例估计气管插管有困难的患者（舌咽关系Ⅲ-Ⅳ级）随机分为导丝支撑引导插管组（A组）和常规导管芯引导插管组（B组）。进行插管对比研究。结果两组一次插管成功率分别是90．0％和67．5％，两组比较差异有统计学意义（P〈0．05），并且A组对咽喉部组织损伤小。结论中心静脉导管导丝支撑引导钢丝螺纹管插管在困难气管插管时应用有效、安全可行。Objective To observe the effectiveness and safety of endotracheal intubation supported by central venous catheter guide-wire using in the difficult endotracheal intubation. Methods Eighty patients who had been estimated difficult to endotracheal intubation（the relation of tongue and pharynx was Ⅲ- Ⅳ degrees ）were divided randomly into two groups, the group of wire-einforced tracheal tube insertion method supported by central venous catheter guide-wire（group A）and the group of insertion method support by routine guide-wire （group B）. Results The successful rate of one time insertion was 90.0% in group A and 67.5% in group B,there was significantly different between two groups （P〈 0.05）. The injury to laryngopharynx of group A was lighter than that of group B. Conclusion Wire-reinforced tracheal tube insertion method supported by central venous catheter guide-wire is effective, safe and feasible.
