摘 要:目的探讨西格列他钠对2型糖尿病患者糖脂代谢、促酰化蛋白(ASP)和瘦素(Leptin)水平的影响及意义。方法选取2型糖尿病患者38例,随机分为两组各19例,治疗组服用西格列他钠16 mg,qd;对照组服用吡格列酮30 mg,qd。治疗12周,检测治疗前后ASP、Leptin、游离脂肪酸(FFA)、空腹血糖(FPG)及胰岛素(Fins)、OGTT2 h血糖(2HPG)及胰岛素(2Hins)、糖化血红蛋白(HbAlc)及血脂水平的变化。结果①治疗后FPG、Fins、2H PG、2Hins、HbAlc、Leptin、FFA均显著下降(均P<0.05),两组间差异无显著性;②治疗后TG、TC、LDL、HDL、ASP变化不明显(P>0.05),但治疗前TG>1.7 mmol.L-1及ASP>75 mmol.L-1的患者,治疗后西格列他钠组TG及ASP降低幅度大于对照组(P<0.05);③治疗后西格列他钠组ASP变化与TG变化正相关(r=0.514,P<0.05),而对照组相关性不明显(r=0.155,P>0.05);④无论治疗前后,ASP与Leptin之间似无明显相关性。结论两种药物均能有效改善糖代谢指标,西格列他钠组ASP变化与TG变化相关可能与PPARα激活有关,ASP与Leptin之间无明显相关性。Objective To investigate effects of chiglitazar sodium on metabolism of lipid and glucose, and levels of acylation-stimulating protein (ASP) and leptin in patients with type 2 diabetes. Methods Thirty-eight patients with type 2 diabetes were randomly divided into two groups, chiglitazar sodium group at the dose of 16 mg once daily or pioglitazone group at the dose of 30 mg daily orally for twelve weeks. ASP, leptin, FFA, FPG, Fins, OGTY, 2 h PG, 2 h ins, HbAlc and serum lipid were measured before and after treatment. Results①Both chiglitazar sodium and pioglitazone significantly decreased the levels of FPG, 2 h PG, Fins, 2 h ins, HbAlc, leptin, FFA after treatment (P〈0.05), no siginificant difference was showed between two gruops.②Treatment of chiglitazar sodium or pioglitazone did not bring about any significant effect on the TG, TC, LDL,HDL ,ASP levels ( P 〉0. 05). In patients with TG 〉 1.7 mmol·L^-1 or ASP 〉75 mmol·L^-1 before the treatment,TG and ASP decreased more significantly after treatment with chiglitazar sodium than those in the control group (P〈 0.05). ③The change of ASP was positively correlated to the change of TG ( r = 0.514, P 〈 0.05) after treatment with chiglitazar sodium, but showed no obvious correlation to the conrol group( r = 0. 155, P 〉0. 05 ). ④No relationship exists between ASP and leptin before and after treatment. Conclusion Both chiglitazar sodium and pioglitazone could improve the glucose metabolism significantly. The association of ASP with TG in chiglitazar sodium group may be due to the activity of PPARa. No relationship existed between ASP and leptin.
