摘 要:目的探讨螺旋CT多种成像技术在颅底骨折诊断中的应用价值,以提高颅底骨折的诊断水平。方法选择经临床和CT确诊为颅底骨折的患者30例,就轴位薄层高分辨率扫描、表面掩盖法三维重建(SSD)和最大密度投影法重建(MIP)的影像表现进行对比分析。结果颅底轴位薄层高分辨率扫描图像显示颅底骨折线25例(83%),与MIP图像相同,SSD显示颅底骨折线23例(77%),除显示骨折线外,轴位薄层高分辨率扫描图像还显示颅内积气9例(30%),筛窦积液18例(60%),蝶窦积液14例(47%),上颌窦积液10例(33%),乳突小房积液8例(27%),额窦积液10例(33%)。结论颅底轴位薄层高分辨率扫描是判断颅底骨折的基本的、可靠的方式,也是三维影像的基础。二者结合应用为颅底骨折的最佳检查方法。Objective To study the application value of spiral CT various techniques on the diagnosis in basicranial fracture, in order to improve the diagnostic levd in basicranial fracture. Methods Thirty cases of basicranial fracture which were diagnosed by clinic data and CT scanning were selected. Comparative study was undergone among thin-section axial helical HRCT scanning,surface shade display (SSD) and maximum intensity projection (MIP) 3D reconstruction technique in all 30 cases. Results Twenty-five cases (83%) of basicranial fracture were diagnosed by thin-section axial helical HRCT scanning as same as MIP 3D reconstruction technique. Twenty-three cases (77%) of basicranial fracture were detected by SSD. Nine cases (30% ) of pneumocephalus, 18 cases (60%) of ethmoid sinus hydrops, 14 ases (47%) of sphenoidal sinus hydrops, 10 cases (33%) of maxillary sinus hydrops, 8 cases (27%) of mastoid cells hydrops and 10 cases (33%) of frontal sinus hydrops were also diagnosed by thin-section axial helical HRCT scanning beside the basicranial fracture. Conclusions The thin-section axial hehcal HRCT scanning is basic and reliable diagnosis technique for basicranial fracture,and it is also the base of 3D reconstruction technique. Combined with two spire CT techniques is the best diagnosis methods for basicranial fracture.
