前S_1抗原在诊断HBeAg阴性慢性乙型肝炎病毒复制中的作用

摘　　要：目的了解前S1抗原(PreS1Ag)与HBeAg阴性慢性乙型肝炎病毒复制的关系及诊断HBeAg阴性慢性乙型肝炎的价值。方法采用酶联免疫吸附试验(ELISA)方法对150例HBeAg阴性慢性乙型肝炎患者血清进行PreS1Ag测定并与血清HBV DNA做对比分析。免疫组化法检测了HBsAg、HBcAg和PreS1Ag在肝组织的表达。结果血清PreS1Ag和HBV DNA检出率为62.7%和66.0%,x2检验,P=0.398,检出率高度符合,无统计学差异。肝组织HBsAg、HBcAg和PreS1Ag的表达阳性率分别为78.0%、58.7%与61.3%。HBsAg表达阳性率最高。PreS1Ag与HBcAg的表达几乎平行,x2检验,P=0.476,无显著性差异。结论PreS1Ag能够反映HBeAg阴性的慢性乙型肝炎患者的是否有病毒复制。Objictive To investigate the role of hepatitis B virus - preS1 （ HBV - preS1 ） in detecting the replication of virus and predicting the prognosis of HBeAg - negative hepatitis B. Methods Using ELISA method, blood sample of 150 HBeAg- negative hepatitis B patients were analyzed about preS1 and HBVDNA comparatively. HBeAg. HBcAg and PreS1Ag in tissue by immunohistochem is try method. Results The coincidence rates of preS1 and HBVDNA were 62.7% and 66% respectively （P = 0. 398 ）. The results of preS1 and HBVDNA detection coincided greatly. Intracellular HBeAg, HBeAg and PreS1Ag account for 78% .58. 7% and 61.3% respectively. Positive rate of expressing HBeAg, PreS1Ag are little less than HBeAg, but near between PreS1Ag and HBcAg. There are no significant difference by Х^2 test （ P = 0. 476）. Contlusion PreS1 can reflect the reiication of HBV sensitively for those who are HBeAg - negative.
