前循环破裂动脉瘤伴颅内血肿的诊断与显微手术治疗(附21例分析)

摘　　要：目的探讨前循环动脉瘤破裂伴颅内血肿的诊断方法和显微手术治疗要点。方法回顾性分析21例前循环动脉瘤破裂合并颅内血肿病人的临床资料,其中前交通动脉瘤6例,后交通动脉瘤4例,眼动脉瘤1例,大脑中动脉瘤10例。均经3D-CTA检查确诊并急诊行显微外科手术治疗。结果夹闭术后随访3～18个月,死亡1例,重残1例,中残2例,其余17例病人恢复良好。结论对前循环破裂动脉瘤并颅内血肿病人应及早确诊并积极采取显微手术治疗。Objective To explore diagnostic methods and microsurgical approaches for ruptured intracranial aneurysm with intracranial hematomas of anterior circulation. Methods Twenty-one cases of emergency operated patients with ruptured anterior circulation cerebral aneurysm with intracranial hematomas were analyzed retrospectively, which included 6 anterior communicating artery aneurysm, 4 posterior communicating aneurysm, 1 ophthalmic aneurysm, and 10 middle cerebral artery aneurysm and all were definitely diagnosed by 3D-CTA and excised in emergency operation. Results After 3 to 18 months of follow-up, 1 patient died, 1 with major neurologic deficits, and 2 with middle neurologic deficits, while the other 17 cases recovered well. Conclusion Ruptured anterior circulation aneurysm with intracranial hematomas should be definitely diagnosed and operated using microsurgical techniques as soon as possible.
