多频听觉稳态反应和短纯音听觉脑干诱发电位在测听中的应用

摘　　要：目的研究多频听觉稳态反应(ASSR)和短纯音听觉脑干诱发电位反应阈(Tb-ABR)与纯音测听(PTA)阈值的差别和相关性。方法选择正常听力耳50例(正常听力组)和听力障碍耳36例(又分为轻、中、重度听力障碍组),分别进行ASSR、Tb-ABR和PTA测听检查,比较两组的ASSR、Tb-ABR反应阈及PTA阈值,同时就ASSR、Tb-ABR反应阈与PTA阈值进行相关性分析。结果ASSR和Tb-ABR反应阈与PTA阈值均有良好的相关性。在听力障碍组中,Tb-ABR的反应阈与PTA阈值接近,而ASSR反应阈与PTA阈值间差值较大。ASSR反应阈与PTA阈值间的相关性要普遍优于Tb-ABR反应阈与PTA阈值间的相关性。结论ASSR和Tb-ABR均为较好的评估行为听阈的客观测听方法。Objective To investigate the differences and relationship between auditory steady-state response （ASSR） threshold, tone burst auditory brain stem response （Tb-ABR） threshold and pure tone average （PTA） threshold. Methods ASSR, Tb-ABR and PTA thresholds were recorded in adults with normal heating（ normal hearing group, 50 ears） and with sensorineural hearing loss （ heating loss group, 36 ears, and were subdivided into mild, moderate and severe hearing loss groups）. ASSR, Tb-ABR and PTA thresholds were compared between the normal heating group and heating loss group, and the relationship between ASSR, Tb-ABR and PTA thresholds was analysed. Results Both ASSR and Tb-ABR thresholds had high correlations to PTA threshold. In the heating loss group, the difference between Tb-ABR threshold and PTA threshold was smaller, while that between ASSR and PTA threshold was bigger. Correlations between ASSR and PTA thresholds were generally stronger than those between Tb-ABR and PTA thresholds. Conclusion Both Tb-ABR and the ASSR can provide reasonably accurate predictions of behavioral threshold.
