北京地区 20682 人群糖尿病筛查及 1566 人 OGTT 的研究

摘　　要：为积极开展对糖尿病的防治，我们在１９９４年北京地区年龄≥２５岁、城市农村各半的２０６８２人群进行标准馒头餐试验，以馒头餐后２小时血糖≥６．７ｍｍｏｌ／Ｌ者１５６６人施以ＯＧＴＴ、胰岛素、血脂、尿白蛋白及血压（ＢＰ）、ＥＣＧ检查。结果表明：北京地区糖尿病（ＤＭ）患病率为３．４４％，比１９８０年增高４．８倍，糖耐量减低（ＩＧＴ）患病率为３．２６％，北京近郊和远郊农村ＤＭ和ＩＧＴ患病率都明显高于城区（Ｐ＜０．００１）。ＢＭＩ≥２５和ＢＰ＞１８．７／１２．０ｋＰａ（１４０／９０ｍｍＨｇ）者ＤＭ和ＩＧＴ的患病率明显高于ＢＭＩ＜２５和ＢＰ≤１８．７／１２．０ｋＰａ者，患病率随年龄增加呈等倍级数递增。本次调查而诊断的ＤＭ组中，已并发冠心病者占２６．９６％，并发早期糖尿病肾病者占３２．８％。In 1994, 20682 residents, randomly seleeted and over 25 years of age, were screened in Beijing area for diabetes mellitus. Half of them were from rural districts. Those whose capillary glucose concentration was above 6.7mmol/L 2 hours after 100g steam_bread were subjected to 75g oral glucose tolerance test. ECG examination was also carried out in these people together with the measurement of blood pressure and determination of plasma insulin, lipids and urine albumin excretion. The results showed:(1)The prevalence of diabetes mellitus(DM) in Beijing area was 3.44%, a four_time increase as compared with That of 1980.(2)The prevalence of DM and impaired glucose tolerance test (IGT) in rural district was higher than that in urban(P< 0.001).(3)The percentages of body mass index (BMI)≥25 and blood pressure≥18.7/12.0kPa(140/90mmHg) were higher in diabetic and IGT group. Than this in the control group (P<0.001).(4)The prevalence of DM and IGT increased significantly with aging.(5)In diabetic patients, 26.96% were complicated with coronary heart disease and 32.8% with incipient diabetic nephropathy. It is imperative that the effective measures should be adopted for the prevention and control of diabetes mellitus and its complications.
