INSULIN DEGRADING ENZYME ACTIVITY OF ERYTHROCYTES IN MULTIPLEX DIABETIC PEDIGREES

摘  要:为了探讨红细胞胰岛素降解酶活性(EIDEA)作为NIDDM遗传分析指标的可能性,采用放射酶分析法检测了家族性NIDDM患者45例及其无糖尿病的一级亲属51例,无家族史的NIDDM患者53例及65例正常人的EIDEA。同时检测了空腹血浆胰岛素(FINS),并对部分个体作胰岛素释放试验。结果表明,在家系、非家系NIDDM患者中,EIDEA及FINS高于正常人,而且这二个指标呈负相关。一级亲属中,年龄≥45岁,体重指数≥24kg/m2的亚组的EIDEA和FINS高于正常人相应亚组。接受口服葡萄糖负荷的各组,其EIDEA与血浆胰岛素在各时相均呈负相关。我们认为:①NIDDM患者的EIDEA升高,提示胰岛素降解加速;②中年以上、肥胖的非糖尿病一级亲属的胰岛素敏感性低于正常人;③EIDEA可以作为NIDDM遗传分析的指标。To investigate the possibility of using insulin degrading enzyme activity of erythrocytes (EIDEA)as one of inherited markers of NIDDM, EIDEA was determined with the method of radioassay of enzyme activity in 45 multiplex diabetic patients (with 3 or more NIDDM patients in the family) and their 45 nondiabetic first_degree relatives, 53 NIDDM patients without family history of diabetes, and 65 healthy subjects. Fasting plasma insulin (FINS) was also determined in all subjects. Moreover, we conducted an insulin releasing test on some subjects. The results showed that EIDEA and FINS were increased in both groups of diabetic subjects either with or without family history as compared with the healthy subjects. Moreover, EIDEA was inversely correlated with FINS in two groups of diabetic patients. In the subgroup of first_degree relative with age≥45yr and BMI≥24kg/m 2, EIDEA and FINS were higher than the corresponding values in the healthy subgroup. In all the groups receiving oral glucose load, EIDEA was inversely correlated with plasma insulin at all stages. We consider that ①the rate of insulin degradation in erythrocytes is increased in NIDDM patients; ②the insulin sensitivity of the middle_aged and obese nondiabetic first_degree relatives is lowered as compared with the controls; ③EIDEA can be used as one of inherited markers of NIDDM.
