氟烷和异氟烷麻醉对人体肝细胞超微结构的影响

摘  要:为了观察氟烷和异氟烷麻醉对人体肝细胞超微结构的影响,我们对16例肝功能正常、乙肝表面抗原、抗体均阴性的胃肠道手术的成年患者进行肝活检,结果:光镜检查全麻后两组见肝小叶结构清楚,肝细胞索完好,偶见肝细胞水肿。电镜观察两组均见肝细胞核膜不规则,粗面内质网、光面内质网出现空泡,线粒体轻度肿胀,肝毛细胆管轻度扩张,微绒毛短少。氟烷组次级溶酶体明显多于异氟烷组。吸入氟烷后血浆ALT活性明显升高(P<0.05)。本研究结果表明氟烷麻醉肝细胞毒性大于异氟烷。The effects of halothane and isoflurane on the ultrastructure of human liver cells were observed in 16 gastroenteric operated patients who had normal liver function and negative hepatic antigen and antibody tests The patients were randomly devided into two groups:group 1 and group 2,each consisting 8 cases Group 1 received 1 mol AC halothane and group 2 received 1 molAC isoflurane Results:The light microscope observation showed that the structures of the hepatic lobules were clear and the hepatic cell cords intact in both group 1 and group 2,with edematous hepatic cells occasioally seen The electron microscope observation showed irregular nuclear membranes,vacuolation of both the rough and smooth endoplasmic reticulum,slight mitochondrial swelling,slight enlargement of the biliary capillaries and shortening of the microvilli in both groups of patients The number of the secondary lysosomes in group 1 was significantly greater than that in group 2 The plasma avanine ALT activity after halothane exposure increased significanly( P <0 05) This study showed that the hepatotoxicity of halothane is greater than isoflurane
