摘 要:目的探讨胸腺素α1(Tα1)对哮喘病人γ干扰素(IFN-γ)-白细胞介素4(IL-4)失衡及肺功能的调节作用。方法选取急性期哮喘病人30例(哮喘组),给予Tα1隔日1次皮下注射,每次1mg/kg,疗程1.5月。同时选取健康志愿者26例作为对照组。分别对哮喘组用药前、后(Ⅰ、Ⅱ组)及对照组留取血标本,测定肺第1秒用力呼气容积(FEV1%)及气道黏性阻力(R5%)。采用双抗体夹心酶联免疫吸附法(ELISA)检测上述3组血清中IL-4、IFN-γ含量。结果Ⅰ组血清中IL-4含量与对照组比较差异有显著性(F=194.45,q=16.840,P<0.01)。Ⅱ组血清IFN-γ含量显著增加,与对照组、Ⅰ组相比有显著性差异(F=6714.04,q=98.165、9.849,P<0.01)。Ⅰ组FEV1%较对照组明显降低,差异有显著性(F=76.66,q=12.224,P<0.01);Ⅱ组较Ⅰ组明显升高,差异有显著意义(q=7.343,P<0.01)。Ⅰ组R5%较对照组明显升高,差异有显著性(F=282.40,q=22.592,P<0.01);Ⅱ组较Ⅰ组明显降低,差异有显著性(q=5.876,P<0.01)。哮喘组IFN-γ与FEV1%呈正相关(r=0.485,P<0.05),与R5%呈负相关(r=-0.516,P<0.05);IL-4与FEV1%呈负相关(r=-0.493,P<0.05),与R5%呈正相关(r=0.528,P<0.05)。结论Tα1能使哮喘病人体内IFN-γ/IL-4失衡得到部分纠正并改善肺功能,从而对哮喘病人的气道过敏反应有治疗和保护作用。Objective To investigate the role of Thymosin-α1 (Tα1) for IFN-γ/IL-4 imbalance and pulmonary function in asthmatics. Methods Thirty patients in acute episode (asthma group) and 26 healthy volenteers(control group)were enrolled. For asthma group, Tα1, 1 mg/kg, hypodermical injection, every other clay for 1. 5 month. The serum samples were taken from the patients, before and after treatment(groups Ⅰ and Ⅱ), and from the controls. The conventional pulmonary function FEV1% and respiratory impedance R5% of the three groups were measured. IL-4 and IFN-γ levels in serum samples were measured by ELISA. The results were analysed. Results IL 4 levels of group I were significantly increased compared with those of the controls (F= 194.45,q= 16. 840,P〈0.0]). IFN-γ levels in group Ⅱ were higher than those in the controls and those in group Ⅰ (F= 6 714.04;q=98.]65,9.849,P〈0.01). FEV1% in groupⅠ was lower than that of the controls (F=76.66,q=12.224,P〈 0.0]); FEV1% in group Ⅱ was higher than those in group Ⅰ (q=7.343,P〈0.01]. R5% in group Ⅰ was higher than that of the controls (F=282.40,q=22.592,P〈0.01); R5 in group Ⅲ was lower than the amount in group Ⅰ (q=5.876,P〈0.0]). Correlation analysis showed that the serum IFN-γ level was positively correlated with FEV1% and negatively correlated with R5% (r= 0. 485, - 0.5 ] 6, P〈0.05) ; the IL-4 level was negatively correlated with FEV1% and positively with R5 % ( r = 0.493,0. 528 ; P〈0.05). Conclusion Tα1 can downregulate the IL-4 level and upregulate IFN-γ, so might partly convert the imbalance of IFN- γ/IL-4 and improve pulmonary function in patients with asthma.
