摘 要:目的:联合检测多种尿蛋白对早期糖尿病肾病的诊断价值。方法:用RIA法检测57例糖尿病患者(分为3组:Ⅰ组28例,为正常白蛋白尿组;Ⅱ组22例,为微量白蛋白尿组;Ⅲ组7例,为大量白蛋白尿组)的24h原蛋白、免疫球蛋白,β2微球蛋白、血肌酐、内生肌酐清除率,并进行比较研究。结果:微量白蛋白尿组原白蛋白与免疫球蛋白明显高于对照组(P<0.01),尿β2微球蛋白与正常组差异无显著(P>0.05),微量白蛋白尿组的血肌酐、内生肌酐清除率较对照组无显著升高(P>0.05),但血β2微球蛋白较对照组升高(P<0.05)。结论:尿白蛋白、免疫球蛋白升高是糖尿病肾病早期诊断的敏感指标,优于尿β2微球蛋白。血β2微球蛋白、血肌酐、内生肌酐清除率对糖尿病肾病无早期诊断价值。Objective: To find out which proteinuria plays role in the diagnosis of incipient diabetic nephropathy. Methods: Urinary albumin, immunoglobulin G (IgG), β2-microglobulin (β2-MG), and serum β2-MG,creatinine,endogenous creatinine clearance rate were measured in 57 patients with diabetes mellitus, who were allotted to three groups according to albumin quantity excreted in urine (group Ⅰ,normal urine albumin group, n = 28, albumin< 12mg/24h urine; group Ⅱ, microalbuminuria group, n = 22, albumin 12~ 300mg/24h urine: and group Ⅲ,great quantity urine albumin group, n = 7,alburnin>300mg/24h urine). Results: Urinary albumin IgG, and serum β2-MG were significantly higer in group Ⅱ than in group Ⅰ (P<0. 001, P<0. 05), but no significant differences of urine β2-MG,serum creatinine, and endogenous creatinine rate between the two groups were found (P>0. 05), Conclusion: urinary albumin and IgG are more sensitivc indices in the diagnosis of incipient diabctic nephropathy compared with β2MG. Serum β2-MG,creatinine and endogenous creatinine clearance rate have not diagnostie value for early diabetic nephropathy.
