摘 要:目的:定量组织速度成像技术通过定量分析心肌运动速度,能够有效评价左右心室局域和整体心脏功能。实验利用定量组织速度成像技术评价阿霉素心肌病大鼠的心功能,验证其在心功能评估中的优势及应用价值。方法:将35只雄性Wistar大鼠随机分2组:①阿霉素心肌病组25只:尾静脉注射阿霉素2.5mg/kg,1次/周,连续10周,制备阿霉素心肌病模型。②正常对照组10只:尾静脉注射等量生理盐水。于实验第12周应用VIVID7彩色超声显像仪行M型超声和定量组织速度成像、RM6000型多导生理记录仪进行血流动力学检测评价其左心功能,并作苏木精-伊红染色观察心肌组织学变化。实验中动物处置符合动物伦理学标准。结果:①死亡率:阿霉素心肌病组大鼠12周累计死亡10只,死亡率为40%。死亡原因为充血性心力衰竭;正常对照组无死亡。②心脏超声测定结果:阿霉素心肌病组大鼠左室收缩末期及舒张末期内径大于正常对照组(P<0.01),左室短轴缩短率和二尖瓣环收缩期长轴方向峰值运动速度低于正常对照组(P<0.01)。③血流动力学测定结果:阿霉素心肌病组大鼠左室收缩压、左室内压最大上升及下降速率低于正常对照组(P<0.01),左室舒张末压高于正常对照组(P<0.01)。④病理学结果证实阿霉素心肌病组大鼠符合心肌病样改变。结论:①长期小剂量静脉注射阿霉素能成功建立大鼠阿霉素心肌病动物模型。②定量组织速度成像技术能够敏感、快速、无创性地评价阿霉素心肌病大鼠左心室收缩功能的变化。AIM: Quantitative tissue velocity imaging could effectively evaluate the function of local left and nght ventricles and general heart. In this study, the heart function of rat model of adriamycin-induced dilated cardiomyopathy (ADR-DCM) was evaluated by quantitative tissue velocity imaging, so as to verify the advantages and application values in evaluation of heart function. METHODS: Thirty-five adult male Wistar rats were randomly divided into 2 groups: ①ADR-DCM group (n =25) was injected with 2.5 mg/kg adriamycin intravenously once a week for 10 weeks to establish models of ADR-DCM;②control group (n =10) received an equivalent volume of normal saline alone. M-mode echocardiography, quantitative tissue velocity imaging method, and RM6000 multiscriptor were used to detect and evaluate the hemodynamics of left ventncle (LV)- at 12 weeks after treatment. The pathological change was analyzed by histological HE staining. The animal treatment was all accorded to the ethical standards. RESULTS: ①The cumulative mortality in ADR-DCM group was 40% (10/12) due to congestive heart failure, but no death was found in control group. ② LV end-diastolic diameter and LV end systolic diameter in ADR-DCM group were significantly higher (P 〈 0.01), while LV fractional shortening and longitudinal peak systolic velocities were remarkably lower than control group (P 〈 0.01 ). ③The LV systolic pressure, +dp/dtmax and -dp/dtmax in ADR-DCM group were markedly lower than control group (P 〈 0.01), while LV end-diastolic pressure was higher than control group (P 〈 0.01). ④The pathological examination of ADR-DCM group was consistent with the histological changes of cardiomyopathy. CONCLUSION: ①Adriamycin of long-term and small dose intravenous injection could induce rat models of ADR-DCM successfully. ②Quantitative tissue velocity imaging method can sensitively, rapidly and non-invasively evaluate the left ventricular function of the rat models of ADR-DCM.
