脾大部切除后胸腔残脾形态和功能的远期疗效评价

摘　　要：目的评价门静脉高压症巨脾大部切除后胸腔残脾的形态与功能。方法门静脉高压症巨脾行脾大部切除40例,采用外周血细胞计数,免疫指标检测,99mTc同位素扫描,DSA造影及CT扫描进行评价。结果外周血细胞计数术后明显改善,血清Tuftsin和IgM水平无变化,残脾形态和吞噬功能正常,残脾显像良好,脾肺间形成侧支循环。结论胸腔残脾可长期存活,脾吞噬功能和免疫功能正常。门静脉高压症巨脾保脾术比脾外伤更有临床意义。Objective To evaluate the modality and function of the remnant spleen in thorax after subtotal splencctomy for portal hypertension. Method 40 patients suffered from portal hypertension were operated on by subtotal splenectomy;Peripheral blood cell counting, measuring of immune index, scintigraphic scan of ^99 Tc-PAY, DSA and CT were adopled for evaluation.Result There was evident improvement in peripheral hlood cell counting,with no change in serum Tuflsin and lgM levels after operation. Modality and phagocytosis of remnant spleen were normal.The process of image development was in good condltion.Branch circulation was formed between spleen and lung. Conclusion Remnant spleen in thorax can be survival for a long. term. Splenic phagocytosis and immune function are normal. There is more clinical significance for patients with portal hypertension and liver cirrhosis on reserving splenic operation than splenic trauma.
