摘 要:目的:观察不同类型急性脑梗死(ACI)患者血浆溶血磷脂酸(LPA)水平变化及其与血糖、血脂的关系。方法:测定100例住院ACI患者LPA水平,并按照急性卒中治疗低分子肝素试验(TOAST)病因分型标准分为5个亚型,分析TOAST各亚型LPA水平变化及其与血糖、血脂的关系。结果:本组TOAST各亚型构成比为:大动脉粥样硬化型38%,小动脉闭塞型33%,不明病因型12%,心源性栓塞型11%,其它病因明确型6%。心源性栓塞型LPA水平最高,其余依次为大动脉粥样硬化型、其他明确原因型、不明病因型,而小动脉闭塞型最低。血糖或血脂增高组LPA水平显著高于血糖及血脂均正常组,血糖增高组与血脂增高组间比较无明显差异。结论:ACI患者LPA水平随TOAST亚型的不同而变化,LPA可能在ACI的发生、发展中起一定作用。ACI患者LPA水平与血糖及血脂有关。Objective : To investigate the changes of plasma lysophosphatidic acid(LPA) level in patients with different subtypes of acute cerebral infarction(ACI)and its relationship with blood glucose and blood lipid. Methods ..LPA was measured in 100 ACI patients. All ACI patients were classified into 5 major ischemic stroke subtypes according to the trial of orgl0172 in acute stroke treatment (TOAST) criteria. And then the relationship between changes of LPA level and blood glucose and blood lipid in different TOAST subtypes were analysed. Results.. The percentage of each ischemic stroke TOAST subtypes was as follow., stroke of large artery atherosclerosis 38%, small-vessel occlusion 33%, undetermined etiology 12% ,cardioembolism 11% ,and stroke of other determined etiology6%. Among 5 major stroke subtypes, cardioembolism patients had the highest LPA level, followed by large artery atherosclerosis, other determined etiology and undetermined etiology, respectively. Stroke of small-vessel occlusion subtypes was assosiated with the lowest LPA level. Apparently,the high blood glucose or high blood lipid group had significantly higher LPA level than the normal blood glucose and blood lipid group. There was no statistically significant difference between the high blood glucose group and the high blood lipid group. Conclusions.. LPA level varied with different subtypes of ACI. The LPA might play a role in ACI development. There was a significant correlation among blood glucose, blood lipid and LPA level in ACI.
