摘  要:目的探讨化瘀理肺方对博莱霉素致大鼠肺间质纤维化的治疗作用及机制。方法将大鼠随机分为对照组、模型组、中药组、激素组,采用博莱霉素经气管内插管注入方法(5 mg/kg)建立肺间质纤维化模型,对照组注入生理盐水(0.25 mL/只),从造模后的第2天起中药组给予化瘀理肺方药物灌胃(13410 mg/kg);对照组、模型组给予生理盐水灌胃(2 mL/只);激素组给予氢化可的松琥珀酸钠腹腔注射(25 mg/kg),于给药后的第14、28天分别处死,称取肺重计算肺系数;取肺组织行苏木素-伊红(HE)染色观察病理变化情况;免疫组化法测定肺组织中TGF-β、Smad7蛋白的表达情况。结果进入结果分析的大鼠44只:①大鼠纤维化程度比较:模型组肺泡炎和肺纤维化程度较重,且胶原纤维沉积较多;中药组可见炎性细胞较少,肺泡结构基本正常,肺泡炎与肺纤维化较轻;激素组肺泡炎及纤维化减轻不明显。②TGF-β、Smad7蛋白表达量比较:中药组TGF-β蛋白表达明显低于模型组和激素组;Smad7蛋白表达明显高于模型组和激素组。结论化瘀理肺方可明显减轻大鼠肺泡炎和肺纤维化的程度,可能通过促进Smad7蛋白的表达从而反馈抑制TGF-β蛋白的表达发挥作用。Objective To evaluate the therapeutic effects and mechanism of Huayulifei on blemycin-induced pulmonary interstitial fibrosis in rats. Methods Rats were randomly divided into four groups: the control group, the model group, the Huayulifei group and the hydrocortisone group. Pulmonary fibrosis was induced by blemycin(5 mg/kg) in the Huayulifei and the hydrocortisone groups, while 0.9% saline was given to the control group. On the second day, Huayulifei(oral, 13 410 mg/kg)was given to the Huayulifei group, intraperitoneal Hydrocortisone sodium succinate(25 mg/kg)to the Hydrocortisone group, and oral saline to the control and model groups. Rats were killed atl4 and 28 days after bleomycin. The lungs were isolated and the lung coefficients were calculated. Hematoxylin and eosin-staining, and expressions of TGF-β and Smad7 proteins were determined by technique. Results: Forty-four rots were involved in the analysis. ① The air-sacculitis and fibrosis of the lungs were fairly severe and much deposition of collagen fibers was found in the model group. There were few inflammatory cells, the alveoli structure was nearly normal, and most of the air-sacculitis and fibrosis of lungs was mild in the Huayulifei group. The alleviation of air-sacculitis and fibrosis of the lung were not significantly decreased in the hydrocortisone group. ② Expression of TGF-β protein was significantly lower in the Huayulifei group than in the model and hydrocortisone groups; expression of Smad7 protein was significantly higher in the Huayulifei group than in the model and hydrocortisone groups. Conclusion Huayulifei can ameliorate blemycininduced pulmonary fibrosis in rats probably by improving the expression of Smad7 in lung tissues and decreasing the expression of TGF-β protein by feedback inhibition.
