冠心病心绞痛患者PAPP-A、GMP-140的变化及与冠状动脉斑块形态的关系

摘　　要：目的观察稳定型和不稳定型心绞痛患者血清妊娠相关蛋白-A(PAPP-A)和血小板α颗粒膜蛋白(GMP-140)的变化,及其与动脉粥样硬化斑块稳定性的关系。方法选取行冠状动脉造影术稳定型心绞痛患者33例和不稳定型心绞痛患者31例。30例健康体检者作为对照组。用ELISA法定量检测外周血清中PAPP-A和GMP-140。对所有冠脉造影显示狭窄≥30%的病变进行影像学分析,对斑块形态进行分型。结果不稳定型心绞痛患者血清中PAPP-A和GMP-140水平均较正常对照组增高(16.75±2.37vs5.37±1.16mU/l;13.57±2.66vs3.82±1.59pg/ml,P<0.01,respectively);同样较稳定性心绞痛患者增高(16.75±2.37vs6.27±2.73mU/l;13.57±2.66vs4.96±2.54pg/ml,P<0.01,respectively)。尽管稳定型心绞痛组PAPP-A和GMP-140较正常对照组增高,但差异无统计学意义(P>0.05)。存在不稳定型斑块的心绞痛患者血清中PAPP-A和GMP-140水平均较稳定性斑块患者升高。结论PAPP-A和GMP-140在不稳定型心绞痛患者外周血中明显升高,可能与斑块的不稳定性有关,可作为反映动脉粥样斑块不稳定的标志。Objective To study the serum levels of PAPP- A and GMP- 140 in patients with stable and unstable angina pectoris, and to explore the relationship between PAPP - A and GMP 140 with the stability of coronary plaque. Methods Thirty- three patients with stable angina pectoris and 31 patients with unstable angina pectoris were enrolled into the study, and 30 healthy subjects were served as control. Serum levels of PAPP- A and GMP - 140 were detected by ELISA. The morphology and plaque stability were determined with coronary angiography. Results As compared with the control group, the serum levels of PAPP - A and GMP - 140 were increased significantly in patients with unstable angina pectoris （ 16.7.5 ± 2.37 vs .5.37 ± 1.16 mU/I, 13..57 ± 2.66 vs 3.82 ± 1.59 pg/ml; P〈 0.0l ） ; similar results were found when compared with patients with stable angina pectoris （ 16.7.5 ± 2.37 vs 5.27 ± 2.73 mU/l, 13.57 ± 2.66 vs 4.96 ± 2..54 pg/ml; P〈 0.01 ）. The serum levels of PAPP- A and GMP- 140 were increased in patients with unstable plaque as compared with those with stable plaque, but no statistical significance was found. Conclusions The serum levels of PAPP - A and GMP - 140 are increased significantly in patients with unstable angina pectoris, and they could be regarded as the two serum markers to evaluate the stability of coronary plaque.
