摘　　要：目的探讨湖南地区中南大学三所附属医院肠杆菌科细菌产CTX-M型超广谱β-内酰胺酶（ESBLs）的检出率及其基因型。方法采用多重PCR法对产ESBLs的142株临床分离株进行CTX-M酶基因检测,然后用组特异性引物对CTX-M酶整个开放阅读框进行扩增,PCR产物测序并确定其基因型。结果湖南地区中南大学三所附属医院肠杆菌科细菌产CTX-M型ESBLs检出率为76.8%（109/142）;组特异性PCR产物测序后发现有三种基因型,分别为CTX-M-15、CTX-M-3和CTX-M-14。结论本地区CTX-M型ESBLs检出率高,共检出三种CTX-M酶基因型,这是在中国大陆首次发现CTX-M-15型ESBLs。Objective To investigate the examination rate and the subtypes of CTX-M-type extended-spectrum β-1actamase （ESBLs） in Enterobacteriaceae isolates collected from three affiliated hospitals of Central South University in Hunan province. Methods The clinical isolates of 142 CTX-M type ESBLs producing Enterobacteriaceae were confirmed by multiplex PCR. The whole ORF （ open reading frame） of CTX-M enzyme was amplified by specific primers, the PCR products were sequenced by automatic sequencer and the subtype of the gene was determined by blast searching. Results The examination rate of CTX-M ESBLs was 76. 8% （109/142） and the subtypes of CTX-M enzymes were CTX-M-15, CTX-M-14 and CTX-M-3 in Enterobacteriaceae in this region. Condusions Multiple subtypes of CTX-M enzymes were detected in this region. This is also the first report of CTX-M-15 in China.
