人工补片结合肌皮瓣修复巨大全层腹壁缺损

摘　　要：目的探讨应用膨体聚四氟乙烯（expanded polytetrafluoroethylene,e-PTFE）补片和复合补片（composix mesh）结合肌皮瓣修复巨大腹壁缺损,预防肠粘连、切口疝的临床效果。方法我科1998-06-2006-01先后应用e-PTFE和复合补片,结合肌皮瓣或皮瓣修复了4例因肿瘤切除和外伤所致的巨大腹壁缺损。腹膜缺损面积最大为10 cm×13 cm,最小6 cm×8 cm。并分别应用腹直肌肌皮瓣、阔筋膜张肌肌皮瓣和腹股沟皮瓣覆盖人工补片。结果手术区愈合良好。随访7个月-2.5年,未出现肠粘连、切口疝和人工材料外露。患者均能正常生活,可从事一般的劳动。结论应用含e-PTFE的人工补片结合（肌）皮瓣移植修复巨大腹壁缺损,具有良好的临床效果,并可预防肠粘连和切口疝的发生。Objective To evaluate the clinical effect of artificial meshes（expanded polytetrafluoroethylene,e-PTFE） and composix mesh on restoration of large abclominal wall defects Methods Four cases of large defect of abdominal wall caused by abdominal wall tumors and injury were repaired by the artificial meshes combined with myocutaneous flaps. Results The patients were followed up for 7 months to 2.5 years,and there were no such postoperative complications as incisional hernia,bowel adhesion and exposure of the prosthetic materials.Conclusion The combination of the artificial meshes containing e-PTFE and myocutaneous flaps is a reliable and effective method for repairing the large defects of abdominal wall.
