摘  要:目的探讨超排卵周期中妊娠相关血浆蛋白-A(PAPP-A)与卵泡刺激素(FSH)和黄体生成素(LH)对卵泡发育是否有协同效应。方法对27例因输卵管因素行体外受精-胚胎移植(IVF-ET)的患者,给予相同的超促排卵方案,采用逆转录-聚合酶联反应(RT-PCR)和酶联免疫吸附试验(ELISA)法检测患者取卵时卵泡液中PAPP-A水平;化学发光免疫法检测用药前(月经第2天)、用药后(hCG日)血清FSH和LH水平。按取卵时卵泡平均直径≥14 mm数目将卵巢反应分为3型:低、中和高反应型。结果卵泡液PAPP-A水平高、中反应型显著高于低反应型,差异有显著性(P<0.01,P<0.05);血清FSH、LH水平高、中反应型均显著高于低反应型,差异有显著性(P<0.05);PAPP-A、FSH、LH三者呈正相关关系。结论超促排卵周期中,PAPP-A和促性腺激素在促进卵泡发育中可能有协同作用。Objective: To explore the roles of pregnancy-associated plasma protein-A (PAPP-A) and gonadotropin in the development of ovarian follicle during superovulation cycles. Methods: 27 patients who underwent IVF-ET because of tubal infertility were treated with the same superovulation regime. These patients were divided into three groups (low, moderate and high reactive group) according to the number of ovarian follicles with diameter ≥14 mm. PAPP-A level of the follicular fluid was measured with RT-PCR and ELISA on the day of oocyte pick-up (Opu). Serum FSH and LH levels were analyzed by radioimmunoassay before and after superovulation. Results: As compared to that in low reactive group, PAPP-A level of the follicular fluid in moderate and high reactive groups was markedly elevated (P〈0. 01 ,P〈0. 05, respectively). Similarly, serum FSH and LH levels in moderate and high reactive groups were also higher than those in low reactive group (P〈0.05). Positive correlation was found among follicular fluid PAPP-A, serum FSH and LH. Conclusion: PAPP-A and gonadotropin may coordinate to promote the development of follicles during gonadotropin stimulation in IVF-ET program.
