花粉过敏性哮喘的免疫学诊断研究

摘　　要：对４８例与花粉过敏有关的外源性哮喘病例进行过敏史、病史、体格检查，结合花粉变应原支气管激发试验（ＢＰＴ）、皮肤挑刺试验（ＳＰＴ）、特异性ＩｇＥ（Ｓ－ＩｇＥ）及特异性ＩｇＧ（Ｓ－ＩｇＧ）检查。结果ＢＰＴ阳性率为７０．９％（３５／４８），ＢＰＴ阳性组的ＳＰＴ、Ｓ－ＩｇＥ以及Ｓ－ＩｇＧ阳性率分别为１００％，７１．４％，６２．９％，明显高于ＢＰＴ阴性组。ＳＰＴ，Ｓ－ＩｇＥ，Ｓ－ＩｇＧ与ＢＰＴ的符合率分别为９３．８％，７５％，６６．７％。Ｓ－ＩｇＥ与Ｓ－ＩｇＧ符合率为７０％。提示：以上免疫学指标在诊断花粉过敏性哮喘方面具有明显的特异性和敏感性。The history of pollen sensitivity and medical history, physical examination, allergen bronchial provocation test(BPT), skin prick test(SPT), specific IgE(S IgE) and specific IgG(S IgG) were examined in 48 asthmatic patients who were suspected of relation to pollen. The results showed that the BPT was positive in 70 9% patients. of them, the positive rate of SPT,S IgE,S IgG were 100%, 71.4%,62.9% respectively, significantly higher than that of BPT negative group. The agreement between the BPT and SPT,S IgE,S IgG were 93.8 %,75%,66.7% respectively. The agreement between S IgE and S IgG was 70%. We conclude that BPT and some specific immunological findings have obviously etiologic specificity and sensitivity.
