不横断颈前肌群甲状腺切除术与传统甲状腺手术的比较

摘　　要：目的：评价不横断颈前肌群甲状腺切除术的应用价值。方法：回顾分析2001年1月～2004年1月,采用不横断颈前肌群甲状腺切除术治疗的150例（A组）临床资料,并于1997年1月～2002年6月,采用传统手术方法治疗的甲状腺肿瘤162例（B组）情况进行比较。结果：A组与B组手术时间、失血量、和住院时间比较分别为：（40.8±10.6）min和（66.9±18.4）min,（41.4±12.5）ml和（74.5±16.4）ml,（4.4±0.7）d和（6.3±0.7）d,两组之间差异有统计学意义（P〈0.05）。两组病例均随访1～3年,切口粘连率分别为1.33%（2/150）和12.9%（21/162）,两组之间差异有统计学意义（P〈0.05）。A组切口平整、术后疼痛轻、颈部切口周围皮肤感觉无明显异常。结论：不横断颈前肌群甲状腺切除术是治疗甲状腺肿瘤创伤较小的开放性术式,术中出血少,术后疼痛轻,简单安全,疗效可靠。Objective：To evaluate the clinical value of thyroidectomy without transection of anterior jugular muscles.Methods：The clinical data of 150 patients who underwent thyroidectomy without transection of anterior jugular muscles for thyroid tumours from January 2001 to January 2004（group A） were retrospectively compared with those of 162 patients who underwent conventional thyroidectomy from January1997 to June 2002（group B）.Results：The operative time,blood loss,and hospital stay between group A and group B were （40.8±10.6）min and（66.9±18.4）min,（41.4±12.5）ml and（74.5±16.4）ml,（4.4±0.7）d and（6.3±0.7）d,respectively.There were significant difference between group A and group B（P〈0.05）.The patients of two groups were survived for 1-3 years,There were significant difference in the incision adhesion rate between group A 1.33%（2/150） and group B 12.9%（21/162） （P〈0.05）.Compared with group B,the skin incision was more smooth,less pain after operation and less hypesthesia or paresthesia than that in group A.Conclusion：Thyroidectomy without transecition of anterior jugular muscles is a minimal invasive procedure for thyroid tumours,which gives little blood loss,mild postoperation pain and is simple,safty and effective.
