产后出血对乳汁分泌影响的临床观察

摘　　要：目的：探讨产后出血对乳液分泌的影响。方法：选择52例产后24h内出血量≥1000ml者为观察组,随机抽样同期74例产后失血〈400ml者作为对照组,观察两组住院期间出现泌乳的始动时间,以及两组产后泌乳量的关系。结果：对照组67.6%泌乳时间在产后第2d内,而观察组59.6%的泌乳时间在产后第3d内;对照组产后1个月内68.9%母乳是足够的,而观察组产后1个月内母乳足够者仅30.8%;至产后9个月,观察组无乳者达40.4%,对照组为17.6%（P〈0.01）。结论：产后出血者出现泌乳时间延迟,泌乳量少。为了促进母乳喂养,提高出生人口素质,积极预防和控制产后出血是我们医护人员的重要责任。Objective：To probe into the influence of postpartum hemorrhage on milk - secretion. Methods：The parturient of 52 cases,whose postpartum hemorrhage was 1000 ml or more after 24 hours＇ delivery,were chosen as observation group. The parturient of 74 cases at the same period ,whose postpartum hemorrhage was less than 400 ml ,were chosen as the control group. The initiation times of lactation for these two groups during the hospitalization period were observed and also the relative factors influencing on the quantity of lactation in two groups were observed. Resuits：67.6% of parturient in control group secreted milk 2 days after delivery. And 59. 6% of parturient in observation group secreted milk 3 days after delivery. 68.9% of parturient in control group had sufficient breast milk within 1 month after delivery and the observation group was 30. 8%. When 9 months after delivery,40. 4% of parturient in observation group had had no milk - secretion while those in control group was 17.6% （P 〈 0. 01 ）. Conclusion：The milk - secretion will be delayed with less quantity for the parturient with postpartum hemorrhage. In order to accelerate the breast feeding and improve the quality of newborn population. It is the medical personnel＇ s important responsibilities to actively prevent and control the postpartum hemorrhage.
