对高血压病及其伴抑郁症患者血小板功能活化的临床研究

摘　　要：目的：观察单纯高血压病及高血压病伴抑郁症患者的血小板活化功能。方法：①对确定抑郁症的高血压病患者计算其在我病房的构成比。②应用流式细胞仪（FCM）对49例确诊为高血压病不伴抑郁症的患者和49例高血压病合并抑郁症的患者及49名无高血压及高血压病史的健康体检者进行血小板活化功能的检测,测定血小板活化依赖性颗粒膜蛋白（CD62p）,血小板质膜糖蛋白（CD42a？CD42b）,血小板糖蛋白GPⅡb/GPⅢa复合物的值。结果：①高血压病合并抑郁症的患者在我病房的发生率为45.5%,其中轻度抑郁为15.00%,中度抑郁为21.17%,重度抑郁为9.33%。②高血压病合并抑郁症组患者CD62P的均值、GPⅡb/GPⅢa复合物的均值较单纯高血压病组升高,且差异有统计学意义,P〈0.05;高血压病合并抑郁症组CD42a？CD42b较单纯高血压病组下降,差异有统计学意义。结论：①高血压病合并抑郁症患者在老年病房有较高的构成比。②高血压病患者存在有血小板功能活化,而高血压病伴抑郁症患者血小板的活化较单纯高血压病患者有统计学意义的升高。Objective： Observing the relationships between the coronary artery disease with anxiety -depression state and the activity function of platelet. Methods ：The first step：calculated the prevalence of anxiety - depression state in the patients of coronary artery disease. Then we examined the activity function of platelet among the 62 CHD and the 60 CHD with anxiety - depression state to determine CD62p,CD41 ,CD61 ,CD2a / CD2b, GP Ⅱ b/GPⅢ a by FCM. The third step ：60 CHD with anxiety - depression state were divided drug group 30 causes and mental group 30 cases in stochastic process. Observing the CD62p, CD1, CD61, CD2a / CD2b, GP Ⅱ b/GPⅢ a and times of Chest pain, times of angina peetoris, scores of HAMD, after 8 weeks treatment. Results： 1 the prevalence of anxiety - depression state in the patients of coronary artery disease is 67. 5%. 2CD62P,GP Ⅱ b/GP Ⅲa is statistical different between CHD and CHD with anxiety -depression stale;there are statistical diffexent between drug group＇ s CD62P and mental group＇ ; CIM2a / CIM2b is statistical different between CHD and CHD with anxiety - depression state. 38 weeks late,CD1 ,CD61 ,CIM2a / CIM2b,GP Ⅱ b/GP Ⅲa,times of Chest pain,times of angina peetoris,seores of HAMD,there are not statistical different. Conclusion： 1 The activity function of platelet occurred in the patients of CHD with anxiety - depression state. 2 The activity function of platelet can be controlled by anti - anxiety - depression state, either drug group or mental group. 3 Rational treatment is beneficial in CHD with anxiety - depression state.
