肝内胆管细胞癌超声造影与增强CT表现的对比研究

摘　　要：目的探讨肝内胆管细胞癌（ICC）超声造影（CEUS）与增强CT（CECT）的增强表现。方法28例患者中28个经病理证实的ICC病灶接受了CEUS和CECT检查。CEUS采用造影剂声诺维和低机械指数实时超声成像技术。CECT采用造影剂优维显和常规双期增强扫描。结果动脉期,ICC在CEUS和CECT上呈高增强者分别有23个（82.1%）和19个（67.9%）,低增强分别为5个（17.9%）和9个（32.1%）（P=0.219）。其中CEUS和CECT表现为周边不规则环状高增强的病灶分别有14个（50.0%）和16个（57.1%）,全瘤高增强者分别有9个（32.1%）和3个（10.7%）（P=0.689）。门脉期,CEUS和CECT分别有1个（3.6%）和9个（32.1%）ICC仍呈高增强,27个（96.4%）和19个（67.9%）为低增强（P=0.008）。CEUS和CECT术前正确诊断的分别为23例（82.1%）和19例（67.9%）（P=0.289）。结论ICC在CEUS和CECT上动脉期表现相仿,但门脉期CEUS上病灶增强消退倾向比CECT上明显。两者诊断能力无显著差异,CEUS可用于定性诊断ICC。　Objective To compare the enhancement pattern of intrahepatic cholangiocarcinoma （ICC） between contrast-enhanced sonography （CEUS） and contrast-enhanced computed tomography（CECT）.Methods Twenty-eight ICC lesions in 28 patients were evaluated by CEUS and CECT.CEUS was performed with contrast agent of SonoVue and low mechanical index contrast specific mode.CECT was performed with contrast agent of Ultravist and a standard biphasic helical CT scanning protocol.Results In arterial phase,the numbers of lesions showing hyper-enhancement on CEUS and CECT were 23（82.1%） and 19（67.9%）,hypo-enhancement 5（17.9%） and 9（32.1%） respectively（P=0.219）.Among the lesions showing hyper-enhancement,the numbers of lesions showing periphery irregular rim-like hyper-enhancement on CEUS and CECT were 14（50.0%） and 16（57.1%）,hyper-enhancement of the whole tumor 9（32.1%） and 3（10.7%） respectively（P=0.689）.In portal phase,the numbers of lesions showing hyper-enhancement on CEUS and CECT were 1（3.6%） and 9（32.1%）,hypo-enhancement 27（96.4%） and 19（67.9%） respectively（P=0.008）.CEUS and CECT correctly diagnosed 23（82.1%） and 19（67.9%） lesions prior to operation （P=0.289）.Conclusions The enhancement pattern of ICC on CEUS was consistent with that on CECT in arterial phase,whereas in portal phase ICC faded out more obviously on CEUS than on CECT.CEUS has similar diagnostic efficacy in comparison with CECT.
