摘 要:目的:探讨2型糖尿病患者血清脂联素(APN)水平与尿微量白蛋白之间的关系。方法:入选2型糖尿病86例,正常对照组(C)20例。检测血清脂联素(APN)、空腹血糖(FBG)、血脂、胰岛素抵抗指数(HOMR-IR)、24h尿白蛋白。根据24h尿白蛋白将糖尿病组分为糖尿病正常白蛋白尿组(NAU)60例和糖尿病伴微量白蛋白尿组(MAU)26例。结果:C组的血清APN水平最高而MAU组最低,C组与MAU组比较有显著差异(P<0.05)。Spearman相关分析显示,脂联素与24h尿白蛋白(r=-0.268,P=0.016)、FBG(r=-0.231,P=0.039)呈负相关。校正FBG后,C组APN水平仍然高于MAU组(P<0.05)。结论:2型糖尿病伴有微量白蛋白尿的患者血清APN水平下降,低脂联素血症与微量白蛋白尿密切相关。Objective:To explore the relationship between serum adiponectin(APN) level and microalbuminuria with type 2 diabetes. Methods: Eighty six patients with type 2 diabetes and twenty control subjects (C) were analyzed for serum adiponectin by EIASA. Fasting blood glucose, serum lipids, insulin resistance index (HOMR-IR) and 24h urinary albumin were measured in all subjects. The patients with type 2 diabetes were separated into normalalbuminuria(NAU) and MAU group based on 24 h urinary albumin. Results:The result showed the serum APN level was significantly higher in group C than group that of MAU (P〈0.05). Spearman correlation analysis showed adiponectin was negatively associated with 24 h urinary albumin (r = -0. 268,P= 0. 016) and FBG(r= - 0. 231 ,P= 0. 039). After the adjustment of FBG, the serum APN level was still higher in group C than that of group MAU(P〈0.05). Conclusion:The result suggests that the serum adiponectin level is decreased in type 2 diabetes with MAU. Hypoadiponectinemia may be associated with MAU.
