摘  要:目的:通过2型糖尿病患者不同胰岛素治疗方案干预前后血糖波动对1,5-脱水山梨醇(1,5-AG)的影响研究分析,探讨1,5-AG能否作为监测糖尿病糖代谢异常状态以及反映高血糖波动的指标。方法:将HbA1c>7.0%的64例2型糖尿病患者随机分为2组,分别给予预混人胰岛素(诺和灵30R)、预混人胰岛素类似物(诺和锐30特充)治疗,比较两组患者治疗前后HbA1c、FA、1,5-AG变化的差异。结果:在给予不同胰岛素治疗后2周后,1,5-AG水平即明显升高,预混人胰岛素类似物组1,5-AG水平高于预混人胰岛素组(17.5±6.9mg.L-1对10.0±6.6mg.L-1,P<0.01),而两组HbA1c控制水平无显著差别(6.9%±0.9%对7.1%±0.8%,P>0.05)。结论:预混人胰岛素类似物对比普通的预混人胰岛素制剂,在相同HbA1c控制水平下,2hPG更接近正常,1,5-AG水平升高更明显,1,5-AG更特异性地反映高血糖。Objective:To study the effects of different insulin treatments on serum 1,5-AG, and to investigate the ability of 1, 5-AG measurements to monitor glycemic control and the postprandial hyperglycemia in type 2 diabetes. Methods: Sixth four patients with type 2 diabetes were randomly assigned into two groups randomly. Group A was treated with biphasic insulin analog (Aspart 30), while group B was treated with biphasic human insulin (Novolin 30), serum 1, SAG, HbAlc, FA, and PPG were measured at baseline and at 2, 4 weeks and 3 months. Results: Serum 1,5 AG was recovered significantly when the pa tients were treated insulin therapies in 2 week, (5.78± 6. 17 mg·L^-1 vs 9. 16 ± 6. 92mg·L^-1). The biphasic insulin analog group had higher 1,5-AG levels (17.5 ± 6.9 mg·L^-1 vs 10.0 ±6.6 mg mg·L^-1 ), while the HbAlc levels were equal. Conclusion:The biphasic insulin analog group had higher 1,5-AG levels than biphasic human insulin group while the HbAlc levels were equal. 1,5-AG measurements reflect more efficiency on the postprandial hyperglycemia.
