超声引导水压灌肠治疗小儿肠套叠多因素分析

摘　　要：目的探讨超声引导水压灌肠复位治疗小儿肠套叠的影响因素,为临床治疗提供参考依据。方法回顾分析149例肠套叠患者资料。按水压灌肠复位成功与否将患者分成两组,分析两组患者的性别、月龄、病程长短、有无呕吐和血便、有无腹腔和套筒内积液、套筒位置和大小等影响因素。结果两组中性别、月龄与病程长短三者构成比无统计学差别,而呕吐、血便、腹腔和套筒内积液、套筒位置、大小等因素,两组间差异有显著性。Logistic回归模型分析提示能纳入模型的有效影响因素为呕吐、腹腔积液、套筒内积液、套筒位置。其作用大小依次为:套筒内积液(OR=41.67)>腹腔积液(OR=31.25)>呕吐(OR=10.42)>套筒位置(OR=8.13)。结论套筒内或腹腔内积液、呕吐以及套筒位置是影响水压灌肠复位肠套叠成功的主要因素。Objective To evaluate the risk factors of Ultrasound- guided hydrostatic reduction of intussusception in children. Methods We retrospectively reviewed the medical records and sonographic examinations of 149 eases with intussusception. Tne sex, age, duration of symptoms, the presence of vomit or hematoehezia,the presence of free peritoneal fluid and fluid inside the intussusception, the initial location of the intussusception, as confirmed by sonography,along with the time of hydrostatic reduction were statistically analyzed to determine their effect on outcome.Results Tne sex,age and the duration of symptoms had no significant difference in the result of hydrostatic reduction. The success rate was significantly lower among intussuseeptions with the presence of vomit or hematochezia, free peritoneal fluid or contained entrapped fluid, and located in the left side of the abdomen （ P 〈 0.05 ）. Of those risk factors, the statistic analysis using Logistic regression model indicated that the importance was the present of fluid inside the intussusception（ OR = 41.67） 〉 the free peritoneal fluid （OR = 31.25 ） 〉 vomit （OR = 10.42） 〉 the initial location of the intussusception（ OR = 8.13 ） in turns. Conclusion Tne present of fluid inside the intussusception and the free peritoneal fluid, the presence of vomit, the initial location of the intussuseeption are the more important risk factors of Ultrasound - guided hydrostatic reduction of intussusception.
