摘  要:目的探讨自身免疫性甲状腺疾病(AITD)发病的始动原因,明确AITD Th1/Th2相关细胞因子群谱的特征。方法用逆转录酶链聚合反应方法检测IFN-γ、TNF-α、IL-2、IL-4、IL-6、IL-10等6种细胞因子的mRNA表达。结果与正常对照组相比,Graves病(GD)和桥本甲状腺炎(HT)细胞因子表达率明显增加;GD Th2类细胞因子IL-4和IL-6总表达频率明显高于Th1类细胞因子IFN-γ、TNF-α和IL-2表达频率,他巴唑治疗前后GD细胞因子Th2强势的表达特征没有明显变化,但总的细胞因子的表达频率下降;HT Th1类细胞因子IFN-γ、TNF-α和IL-2表达频率高于Th2类细胞因子IL-2、IL-4、IL-6、IL-10的表达频率。结论GD细胞因子表达向Th2方向漂移,以体液免疫为主;他巴唑治疗后的GD细胞因子谱没有明显改变,而HT细胞因子表达则呈现Th1强势,以细胞免疫为主。[ Objective ] Studying the initial pathogenesis of autoimmune thyroid disease ( AITD), and clarifying the Tb1/Th2 cytokine profile in AITD. [ Methods] The gene expression of IFN-γ,TNF-α,IL-2,IL-4,IL-6 and IL-10 were detected respectively by the method of reverse transcription polymerase chain reaction (RT-PCR). [ Results ] Compared with the control, the expression rates of all cytokines were significantly elevated in Graves'disease (GD) and Hashimoto's thyroiditism (HT). In GD, the expression rates of Th2-1ike cytokines (IL-4,IL-6) were much higher than those of Th1-like cytokines (IFN-γ.TNF-α, IL-2), but no change happened after being treated with methimazole. In HT, the expression rates of cytokines ( IFN-γ, ,TNF-α, IL-2) were higher than those of Th2-like cytokines ( IL-4, IL-6, IL-10 ). [ Conclusion ] The predominant expression of Th2-like cytokines mainly mediating humoral immunity was found in GD, and the expression of cytokine profile was unchanged after being treated with methimazole, while a predominant expression of Th1-like cytokines nrainly mediating cell immune responses was found in HT.
